The fibroids themselves rarely become cancerous, but there is 5-10% chance of endometrial hyperplasia coexisting with the fibroids. It may cause heavy bleeding and after age 40, it may become worse in 25% cases. Biopsy is mandatory to ensure you don't have the type of endometrial hyperplasia which progresses to cancer. Usually the progress is slow if left untreated and for the cancerous change to happen it can take 10-20 years. Best option after the age of 40 is biopsy if periods become heavier (5+ days) and irregular.
