I am 61 and have iga nephropathy for the past 11 years which is being treated with cardace 10 mg, stamlo 5 mg, lipitor 10 mg, and fish oil capsules. I had an ultrasound in june 2019 which showed normal kidneys. In addition I have a benign prostrate (grade 3) - no medication. Bp is around 130/80. My recent test results in july 2021 are as follows: 1. Routine urine examination -no protein or rbcs noticed 2. Total cholesterol - 151 mg/dl, ldl 95 mg/dl 3. Serum creatinine 1.4 mg/dl (lab range 0.72-1.20 mg/dl). In oct 18 it was 1.39, april 19 it was 1.24, and then in nov 2019 this was 1.19 mg/dl. 4. My platelet count has been historically low varying from 128,000 - 150,000, the latest test shows a platelet count of 112,000. 5. Bun (blood urea nitrogen) -12.4 mg/dl 6. Psa - 1.32 ng/ml 7. Albumin in urine - 58.3 mg/l with acr at 122.92 ug/mg (nov 2019 - acr 117.40) 8. Phosphorous 5.3 mg/dl (ref range 2.5-4.5 mg/dl) (nov 2019 - 4.1) 9. Ldh -lactate dehydrogenate: 242 u/l (ref range 135-225 u/l) (nov 2019 - 215) 10. Hs crp 1.68 mg/l (low risk <1.0, avg risk 1.0 - 3.0) 11. Homocysteine 24.73 umol/l (ref range 0-15) 12. Fbs 93 mg/dl, hb1ac 5.8 (nov 2019 - 5.8) my queries are as follows: 1. My creatinine level which was 1.2 mg/dl for the past 1.5 years (i test every 6 months, lab range 0.50-1.2 mg/dl) increased to 1.39 mg/dl in the oct 18 test, decreased to 1.24 in april 19, 1.19 mg/dl in nov 2019 and is now at 1.40 mg/dl in the july 2021 test. Is this a cause for concern with regard to my iga nephropathy condition? I exercise reasonably strenuously 6 days a week for 45 minutes but I avoided exercise for 3 days before the blood test. During the pandemic from march 20 to july 21, I lost 4 kgs from 67 kg to 63 kg. 2. What steps can I take to reduce my creatinine level? 3. My phosphorous and ldh levels (items 8 and 9) show an increase from my last test in nov 2019. My calcium level is within range. Is this a cause for concern? I took a vitamin d capsule 60k one week before the blood test - would that high dose of vitamin d have an effect on my creatinine and phosphate levels? 4. What is the significance of the hscrp and homocysteine levels being above the range? I did an echo cardiogram in july 2019 and the results were normal with an ef of 74% 5. Is the hb1ac level at 5.8 a cause for concern - this is classified as pre diabetic in the report. Thanks.
