My brother aged 45 has severe back pain along with pain on his right thigh. The mri report says that 1. Regeneration spondulitic changes posteirial limbs at l4 superior end plate. 2. Diffuse disc bulge atl3-4 l4-5 levels with annulus tear posterial with central profusion causing canal narrowing. Identification of preexisting nerves in canal bilaterally. Diffuse disc bulge l5-s1 level with annulus tear posterially with posterio central disc extrusion. Extruded disc fragment migrating inferially in canal and causing severe canal narrowing mild neural foremen narrowing no nerve compression. Significant compression of traversing nerve in canal bilaterally mild indentification of existing nerve root in neural foremen bilaterally. What sort of treatment is needed for him?
