Most of your conditions needs attention from a general physician/diabetologist but I can advise you regarding your knee pain. Even your foot swelling probably does not have a musculoskeletal cause, but still I can tell you that persistent swelling in foot region can be due to deranged kft, lft, thyroid profile and issues with heart. Sometimes even chronic protein deficiency can lead to persistent foot swelling. Looking at your age, I would think that if there are no external s/s of inflammation, then knee pain would be due to degenerative changes setting in the joint. Please catch hold of a good physical therapist and do intensive knee - quadriceps and hamstring ex, avoid sitting cross legged and squatting, use a western toilet etc - take adequate calcium and vitamin d as per your body's requirement - daily walks for 2-3 km per day in morning should help you. You can take a short course of a safe anti-inflammatory medication - hot fomentation at home 3-4 times in a day might also help in your knee pain but mainstay remains exercises please send me both knee ( ap & lateral) x rays so that I can see and advise you accordingly.
