Lybrate user, as far as psychiatric medicines are concerned, it is most of the time 'trial and error' method. Meaning, a particular medicine to a particular patient has to be given and followed (under the supervision of the psychiatrists) for a certain period of time to come to a conclusion as to whether it should be continued or replaced by some other meds, depending on the results. So, it is not advisable to change the doctors frequently. Visit the same doc. Who prescribed the previous meds so that he/she can better understand your father's present condition and change the prescription or titrate the doses accordingly. Â

Dear user as your father is diagnosed case of bipolar disorder. 1) he should be on a mood stabilizer preferably lithium or sod valproate. 2) this could also be a case of antidepressant induced switch, I would recommend to stop antidepressant and increase dose of olanzapine along with mood stabilizer.Â

Dear Lybrate user, in my opinion, your father might be showing initial signs of episode of mania which might occur in patients with bipolar disorder who are prescribed antidepressants like fludac. This is called a manic switch and first step to resolve the symptoms would be stop any kind of antidepressant medication and avoid prescribing another like nexito. Please only consult a psychiatrist for this purpose who will assess your father and look at his past treatment before changing medication. Discontinuing fludac and increasing olimelt are the options you can discuss with your psychiatrist. Another category of medications called mood stabilizers can also be added if symptoms are not controlled by stopping fludac and increasing olimelt. Â

Having mood swings is very common with the condition your father has. Stopping fludac may be one way of handling the situation. However your father seems to have been on this medication for a long time and it takes a long time to get out of the system due to its pharmacodynamic properties. I also believe not starting nexito but giving him a higher dose of mood stabilizer will be more helpful. You can consult us for a second opinion. Look forward to hearing from you. Dear lybrate-user, probably your father has gone into bipolar mania. He needs more mood stabiliser and antipsychotic dose. But at 76 years of age, he may be having some medical illnesses or dementia causing these kind of problems. So he needs to be assessed properly by a psychiatrist and physician. No need to change into nexito. With the same medicines, please go for consultations at the earliest. Â
