Came with a problem of persistent hirsutism & was diagnosed as pcod successfully currently pcod is not there as per usg inspite of that hirsutism is persistent old hair follicles not going away but new ones do not come. Was rx c^ 6 months cypokare that got rid of pcod & then 1 month spironolactone 50 mg od. Problem is hirsutism is still present and not going away though there are no new follicles. Please suggest diagnostic and therapeutic approach from this point what could it be. Why older hirsutism follicles are not going away and what has to be done post pcod treatment very recent reports 17 hydroxyprogesterone 1.38 ng / ml serum cortisol 8: 00 a.m. 22.3 (3-21 normal) serum cortisol 8 p.m. I 0.6 (should be less than 50% off a m value) acth 18.3 pg / ml (7.2 - 63.6)u s g is normal post pcod treatment.
