Sir my age is 27 and height is 5 feet 11 inch and weight is 82 kg. I have undergone all pubertal changes like change in voice, body hair, chest hair except very less beard like moustache and chin and my penis size and testicle size is small than normal. Penis is around 4.5 inch .from the age of 22 I was noticing a reduction in my testis size due to which I went to a urologist who ordered my ultrasound of testis and fsh and lh and serum testosterone.in 2017, ultrasound of the testes showed as volume of one testicle is 8 cc and other testicle is 10 cc and epididymis cyst of 2 mm on left side of the testis. Along with that it showed mildly prominent pampiniform plexus bilaterally. My serum testosterone was around 2.98 ng/ml (range 3 to 10. Fsh values were 6.63 ng/ml and lh was 4.58 ng/ml. Both were normal range at the time. Ordered my semen analysis which shows no sperm that is azoospermia. And fructose was positive in that test. My transrectal ultrasound which showed prostate is normal in size around 13.5 cc all all the seminal vesicles are normal. Finally he asked me to go for fnac of the testis. Then he again ordered semen analysis which again showed no sperm. After that I did my serum testosterone, lh and fsh many times upto 2018 but every time fsh was in same range around 6miu/ml. And lh was decreasing and in 2018 it was 2.80miu/ml (range 1 to 9).after that on 24 aug 2020, I again did my total testosterone i.e 2.53 ng/ml (range 3 to 10, my lh was 2.64miu/ml ,fsh was 8.10miu/ml (range 1 to 14) and prolactin 12 (range less than 20) and semen analysis showed nil sperm. My question is that in primary testicular failure fsh goes above normal range but my serum fsh is still in normal range and lh is going down to lower normal value. Urologist said to me its a case of primary testicular failure but how when fsh is not abnormal?
