Currently smile and contoura both are latest technologies. Lasik was 1st generation in which corneal flap was created with blade and then a excimer laser was used to treat cornea and flap was deposited back. 2nd generation was femtolasik (bladeless lasik) in which now flap is created with precise femtosecond laser and rest procedure is same as lasik. It improved the safety of procedure as laser cut is better than blade cut flap. 3rd generation is - contoura vision - is basically a femtolasik procedure with topography guided treatment. It is marketing name for topography guided lasik procedure from alcon inc. Topography guided lasik is also available from carl zeiss. It is a customised lasik procedure in which your corneal scan is fed to the machine to treat corneal irregularities in better way than femto lasik. In contoura treatment (like in femto lasik), a flap is created with femtosecond laser and then topography guided treatment is done on cornea after which flap is put back. Contoura is gives better results than femtolasik in patients with high astigmatism (cylindrical numbers) and hyperopia (plus numbers). In patients who have spherical number and normal shape of cornea - conoura gives same results as regular femtolasik. It has similar limitations as lasik such as dry eye and flap related complications as flap has to be created in contoura vision too (which is its drawback). Then smile (flapless and bladeless lasik) is 4th generation technology. During smile - neither flap is created or blade is used. So it is currently safest technology today.
