If you are now better as per your depression diagnosis if it was MDD
THEN ONLY aripiprazole AND LAMOTRIGINE CAN BE TAPPERED GRADUALY. If it is bipolar depression then stratigy can be different.there is no long acting combination for more then a day preparation.OD ONCE ADAY IS AVAILABLE.
GRADUAL TAPERING DOWN ONLY TO ADEQUATE DOSE OF venlafaxine od 150 mg or 225 mg may be possible.
I dont advise to do it own your own because self medication can be dangerous.

Believe at age above 60, even systolic blood pressure till 150 is acceptable. Once in a week bp monitoring, mild reduction in salt intake (after monitoring electrolytes balance), and then periodic review with your doctor should help. If you have been using propranolol fro some time, then it is clinical judgement, but pls don't suddenly start it at 80 mg/day, as it can cause sudden fall in blood pressure.

Dear lybrate-user, why do you want to stop venlafaxine before you are relieved of your anxiety and depression. Good that you have not used ativan. Alprazolam also is a similar drug. Use it only when it is absolutely necessary (you need to do your routine work with all these symptoms). Paracetamol may relieve you temporarily - but in the long run, it damages your kidneys. Dose of venlafaxine is very less and it needs to be increased. Once you are on higher dose of venlafaxine then olanzapine can be stopped.

Praxotine is a ssri, it will not make sense to add another ssri. Have you tried snri such as venlafaxine. Switching to snri is potential option. Depending on other symptoms such as level of anxiety, adding bupropion is a good option (unless you have anxiety). Trial of small dose of tricyclic or mood stabilizer are also options.

based on the limited information provided by you, it is not clear whether mirtazapine was the first medicine given to you after you were diagnosed with ocd or it was started after trial of other medications. How were you doing/improvement over mirtazapine? It's always best to cross taper a psychotropic medication rather than stopping abruptly which may otherwise lead to drug withdrawal/loss of effect.
