Mr. Lybrate-user, the tablet mentioned has 4 mg of glimiperide and 1000 mg metformin. This is a high dose particularly if some one is starting the treatment. Plus dose is also dependent on blood sugar levels both fasting and pp. Plus hba1c%. So I need to know that before guiding regarding whether to take that tablet or not. Thanks.

Triglimisave 2 and metformon can not be vompared and sjould not be used as substitute to each other.
Trihlimisave contains 3 medicines and metformin is one of them.

I have been taking glitaris m7. 5/500 one tablet after dinner for sugar control for more than 10 years. This has been banned again in india. My endocrinologist is of the opinion that I should not stop pioglitazone (the reason, it facilitates utilization of insulin in body cells). Secondly, I am taking it in low intensity (7.5 mg). My questions are the following. 1. Does it have harmful effect? 2. Is the side effect related to quantity of pioglitazone one is taking? Meaning, is to safe to take it in low amount as in my case? 3. Though this tablet is banned, glitaris 7.5 mg is available in market (exclusive pioglitazone). Can I take this with one additional tablet glycomet 500 (metformin)? If yes, 4. If combination tablet glitaris m7. 5/500 is unsafe (because it is banned) then how the same in separate form, glitaris 7.5 + glycomet 500, can be safe?

As told by your endocrinologist pioglitazone does help insulin action by reducing resistance to its action. The combination has been banned because it is considered as irrational to have certain "fixed dose" combinations. Because it is difficult to make dosage adjustment in asuch a combination. Looking at your fasting and pp glucose levels, it is clear that diabetes is not at all in control. Ideally in a well controlled diabetes fasting glucose should be < 100 mg & pp 160 to 170 mg and hba1c% < 7, considering your age. It is clear that present medication is not helping, there is a need for upgrading the treatment. I have read the details given. The fasting glucose & HbA1c% are quite high. Therefore, Insulin might have been advised. Plus already there are multiple oral drugs like Glimiperide, Metformin, Acarbose, Vildagliptin & Pioglitazone. WHich means the whole oral range is there.
