Hello, Thanks for the query.
It is difficult give a clear answer whether a SGLT - 2 Inhibitor can be added to existing therapy withput knowing full details of the case. I need to know the following: Fasting, PP glucose levels, HbA1c%, weight, height, serum creatinine & urea levels, eGFR and whethe there is microalbuminura. If kidney function is compromised or eGFR is low, then certain drugs can not be sude including metformin and SGLT-2 Inhibitors. Thanks.

Mr. lybrate-user, Thanks for the query.
When one takes adequate dosage of drugs, plus follows a strictly controlled diet and does regular exercise, blood glucose does get controlled. Therefore, it appears your present treatment is not proper. I need to know following things for giving any suggestions: fasting & PP glucose levels, HbA1c%, present dietary pattern and extent of daily exercise. Looking at your weight and height it is clear that with a BMI of 28.23 kgs/Sq meter (normal < 23 kgs/S...more

Hello, thanks for the query.
I have seen the query. It is incomplete. There is no information on what is the fasting, pp glucose level or hba1c%, or your weight, height, dietary pattern and extent of physical activity. So difficult to give any suggestions. Thanks.

Hello, thanks for the query.
The glucose levels have come down because of medication plus dietary control (if that is being done) plus regular exercise (again if you are doing it). If you stop the medication the glucose levels will again shoot up. Ideally when on appropriate treatment the glucose should be as follows: fasting < 100 mg, pp 14 mg or so. Hba1c% < 6.50%. So please continue the medication as advised plus do follow diet control and regular exercise. Thanks.

Dr. lybrate-user for the query.
The blood glucose levels mentioned are fairly high, suggesting diabetes is uncontrolled. It is clear that present medication is not really helping, there is a need for up gradation of treatment. Ideally in a well controlled diabetesfasting should be < 100 mg, PP 160 to 180 mg in a slightly older person. HbA1c% < 7.
To help achieve better control we need to look at his dietary pattern, extent of daily physical activity (exercise) and the medication. I would...more

Hello, Thanks for the query.
I have seen the details mentioned. It is not possible to give any opinion on your specific question. To give a clear answer I need to know the following: 1)which is the earlier brand of glimiperide + metformin combination was being used? At that time what were fasting & pp levels as well as hba1c%? 2) since when generic drugs are bing used? How often blood glucose is being checked after changing the tablets (brands or generics)? 3) has there been any change in th...more
