There are various reasons of bad breath, like impurities in blood, inefficient digestive system, dry mouth (lack of saliva production), gum disease, cavities, plaque and tartar formation around teeth and gums, kind of diet, improper brushing / cleaning of mouth etc. I would advise you to get a thorough physical and dental examination to rule out the cause of bad breath and to solve it permanently.

Tooth brush to be changed every 2 months. Changing the toothbrush alone donot keeps your teeth clean. All toothpaste have nearly similar ingredients like Abrasives, Fluorides, Surfactants, Antibacterial agents, Flavourants, Remineralizers, Fluoride, Diethylene glycol, Triclosan etc. Only the wrong brushing technique that we follow harms our teeth and gums. Plaque is due to food particles repeated mouthwash twice daily and brushing twice daily flossing once daily is enough.

You may try snowdent toothpaste. It creates mild bleaching effect to whiten your teeth. Or you may make fresh paste by mixing baking soda and water, put it on your tooth brush and brush your teeth (not more that 2 minutes). This home remedy may help you to whiten your teeth. If still doesn't help you then ask your dentist about professional bleaching of teeth.
