Pelvic Inflammatory Disease, also known as PID, is characterized by an infection that occurs in a lady's reproductive organs. Usually this occurs as a result of a complications arising from a STD. The disease causes severe damage to the ovaries, fallopian tubes and the uterus. It is one of the bigger causes of infertility in women. 

Causes-

The cervix which is the entry to the uterus has the function of stopping bacteria that are present in the vagina from spreading to the other organs. Once infected by a STD, the cervix can no longer perform its function of preventing the bacteria from spreading to other organs. PID develops when the infection spreads from the cervix to the upper areas of the genital tract. 

There are certain factors which increase the chances of being affected by PID: 

Being sexually active 
Already affected by STDs such as gonorrhea 
Past history of PID 
Having multiple sexual partners 
Symptoms-

Women may experience pain while urinating and engaging in sexual intercourse, coupled with sore feeling in the stomach along with nausea. Abnormal vaginal discharge and a rise in body temperature are common symptoms as well. 

Treatment-
Once the disease has been diagnosed by a doctor, a treatment plan is devised. The treatment options that you can avail of are: 

Medications: If the symptoms are mild, then the doctor will prescribe antibiotic medications. Usually the antibiotics are administered by mouth unless the symptoms are severe, when the drugs are administered intravenously. Your partner would also require medications if you happen to be diagnosed with PID, so that he/she does not catch it. 
Surgery: Surgery is recommended by the doctor in case of tissue inflammation and the presence of pus (abscess). The aim of surgery is to get rid of the abscess to prevent any further infection. 
In addition to the treatment methods, there are certain precautionary measures that you can adopt to be safe: 

Have protected sex.
Try and stick to one sexual partner.
Visit your gynecologist for regular checkups.

Pelvic Inflammatory Disease - Can STDs be the Reason Behind it?

Precum is nothing but release or ejaculation of the fluid from the penis during sexual stimulation. Normally, it acts as a lubricant during sex. The amount of precum that a normal human body releases may vary from person to person. Suppose the body is releasing the extra amount of precum that means that the body is releasing that amount of fluid which would be considered as the normal release of fluid from the body. The main purpose of the precum is to neutralize the acidic nature of the urethra. Basically, this helps the sperm to be active inside the woman s body. Precum may contain immobile, dead and some weak sperms as well. 

Origin
It comes from Cowper s gland Secretions and the actual ejaculation comes from testicles. Normally, when a healthy man is energized or is aroused then, he pre-ejaculates where the fluid flows through the urethra, the place where the urine passes out. It acts as a lubricant to the female partner s vagina. It also tries to neutralize the acid which reduces the chances of pregnancy. This works as a clear method for enhancing the pregnancy.

Difference between pre-cum and sperm
The basic difference between sperm and precum is that the injection of the sperm into a woman s body would lead to pregnancy while in case of precum, the rate of pregnancy is quite low. Pregnancy is caused only due to sperms which do not originate from the Cowper's gland. Thus, sperm and precum are way more different than each other.

Can precum make a woman pregnant?
There have been several deliberations on the issue of whether precum can make someone pregnant or not. Well, in many situations it has been found that it has the capability of making a person pregnant. But the chances are quite low as the presence of sperm in the precum is comparatively lower which would not lead to pregnancy as the content of the sperm is quite lower and sperm is considered to be imperative for pregnancy. There is quite a thin line of difference between ejaculation and precum as sometimes they might get mixed if the withdrawal method is a bit longer. Even if the withdrawal method is delayed by a few seconds, a woman can get pregnant.

Can it cause STDs?
Normally, people get intimidated by precum because they think that it might cause pregnancy, but instead of pregnancy, there s a high possibility of Sexually transmitted diseases to be caused which is way more dangerous than getting pregnant. Seeing spots of blood during pregnancy is usually a cause for concern. However, it is not uncommon. The light bleeding, which is known as spotting may occur for a number of reasons.

Bleeding during the first trimester

Spotting is most likely to happen during early pregnancy and the reasons could be many, such as:

- implantation bleeding occurs during the first month of pregnancy when the fertilised egg gets attached to the wall of the uterus.

- cervical polyps ( a tumour that form on the surface of the cervix) or yeast infection may also cause episodes of spotting.

- miscarriage may be another possible reason for spotting

- ectopic pregnancy is another cause. It is the condition in which the embryo plants itself in either of the fallopian tubes instead of the uterus.

- molar pregnancy is a rare condition associated with spotting during pregnancy. When this is the case, instead of your baby, an abnormal tissue grows inside the uterus. Sometimes it can be cancerous too.

Bleeding during the second or third trimesters

Spotting may also occur during late pregnancy and the possible reasons are listed below.

- problems related to placenta like placenta previa placental abruption can also result in spotting. These are serious conditions when the placenta either covers the cervix or it gets detached from the uterine wall respectively.

- uterine rupture is the rare and unfortunate event when a scar from a previous surgery (like the removal of fibroid or previous caesarean scar bursts open and the baby slips into the pregnant woman's abdomen.

- premature labour occurs when your body is too eager to deliver the baby, usually one month before the due date of delivery.

- vasa Previa is another rare condition when a baby who is still developing has his blood vessels entangled in the umbilical cord or placenta-crossing the cervix. It is extremely dangerous because the blood vessels may burst open thus causing the baby to bleed and lose oxygen.

Reasons for Spotting During Pregnancy

There are two methods by which abortion can be done:

Medical Abortion - Non-surgical abortion in which medicines are used to induce abortion

Surgical Abortion - Use of transcervical procedures for terminating a pregnancy, including vacuum aspiration, and dilatation and evacuation (D and E).

Surgical or Medical Abortion for Unwanted Pregnancy

If pregnancy is lesser than = 7 weeks; medical method with Mifepristone with Misoprostol is favorable as the success rate is as high as 97%.

If pregnancy is between 7-12 weeks; surgical abortion is the only option.

If pregnancy is beyond 12 weeks - 20 weeks, then the woman needs to be hospitalized and will be given either Mifepristone followed by Misoprostol tablet or only Misoprostol tablets and woman will then abort like a miscarriage in the hospital. If any products are remaining, then it will be evacuated surgically.

Surgical abortion is preferred if patient desires concurrent tubal ligation or IUCD (CuT) insertion.

If a woman fulfills the criteria for selecting either method i.e. before 7 weeks, final choice has to be given to the woman.

Summary of choice between Surgical or Medical abortion in India

lesser than = 7 weeks - Medical method or surgical method

7-12 weeks - Surgical abortion

12 - 20 weeks - Medicines with or without surgical procedure in hospital

Advantages of Medical Abortion

Avoid surgery

Controlled by the woman and may take place at home

No need for admission to the hospital

Usually, well tolerated by women, if properly counselled and motivated

No need to take leave from work, as bleeding can be managed with menstrual pads even while in the office or at work.

Disadvantages of medical abortion

Takes time (hours to days) to complete abortion, and the timing may not be predictable. Women experience bleeding and cramping, and potentially some other side-effects (nausea, vomiting, fever, and shivering). May require more clinic visits than surgical abortion.

Advantages of Surgical Abortion

Quick procedure

Complete abortion easily verified by evaluation of aspirated products of conception

Takes place in a hospital so Sterilization or placement of an intrauterine device (IUD) may be performed at the same time as the procedure. It can be performed under local anaesthesia if woman is properly motivated and counselled

Disadvantages of surgical abortion

Requires instrumentation of the uterus

Small risk of uterine or cervical injury

Timing of abortion controlled by the doctor and hospital

Anaesthesia needs to be administered. Most doctors perform this procedure under short general anaesthesia. A woman needs to be in the hospital for at least 6 hours. Effect of sedation will be there throughout the day.

Methods of Abortion for Unwanted Pregnancy

Polycystic ovary disease or PCOD is a kind of hormonal disorder in which a woman produces a surplus of male hormones like testosterone.
High levels of male hormones produce symptoms like irregular ovulation or no ovulation at all. In simple words, women with PCOD or PCOS don t release a mature egg from their ovaries every month resulting in infertility. PCOD affects one in 10 women worldwide.
