My father was prescribed xarelto 20 mg od. As his creatine is 1.3 and he is having lumber spondylitis so, can he continue xarelto 20, as he is suffering from back pain after started taking it. Can he stop taking xarelto and start warfarin? Pls inform.

Xarelto dosing can be adjusted if there are kidney problems but certainly not contraindicated. However, exact dosing needs to be calculated based on his creatinine clearance (a measurement used to define kidney function). Regarding your question on back pain from Xarelto - if you very sure the pain started after taking the medication or got very worse after taking the medication and it's unbearable, then it's not a bad idea to talk to your doctor who prescribed the medication about switching to other blood thinners such as warfarin. It's not advisable to make that switch yourself.
