PCOS (Polycystic Ovarian Syndrome)

Hi I am Dr. Anita Rajurkar. I am practicing in pitam pura since 1990 and attached to Max hospital pitam pura and Shalimar bagh and my main interest of, main interest is in dealing with highly pregnancies, infertility and non-decent viginal esterifies. I will be talking about Polycystic Ovarian disease. I have chosen this topic because the incidence of Polycystic Ovarian disease is rising remarkably. Why is it rising? Because of our lifestyle changes. We have adopted sedentary lifestyle. We have diet habits are changed. We are eating more of junk foods and even our children at from the very young age, they don’t play out. They are doing indoor activities only and they are eating junk foods. So we have seen that the incidence even at young children that is 10 to 12 years of age also, these children have tendency of having Polycystic Ovarian disease. So as we are seeing incidence of Polycystic Ovarian disease is rising. What is Polycystic Ovarian disease? It is a disease in which patient can have 3, 2 out of these 3 criteria which I am going to tell you. First is an irregular period. Periods are delayed. The come at very regular intervals. Secondly, they have increased type of, male type of hormones. These hormones are called androgens. So because of that patients get hirsutism, that is excessive hair growth over face, chest, back etc. and there acne. Later on as they become adults, in adults there is a problem of infertility that is difficulty in conceiving because of irregular periods. These patients are prompt to have diabetes, later on in life they can have hypertension, they can have sleep apnea, and they can have endometrial carcinoma etc. later on in life. So we have to treat these patients at every stage of life. In adolescent young girls we have to treat for their acne, hirsutism and their irregular periods. In adults treatment is required for infertility and if they conceive then also we have to look for the various complications which can develop their pregnancy in the form of diabetes, hypertension and premature delivery, growth, less growth of te baby because og hypertension and so forth. When they become, later on the life, they are prompt to have cancer of the body of the, cancer of the uterus. They can have, because of hypertension they can have various problems and they are also prompt to have something called metabolic syndrome, in which the cholesterol level increased and they are prompt to have heart problems. So we have to counsel these patients that they have to maintain active life style and diet control throughout their life to prevent and to, so that they don’t have these complications in their life. Now PCO patients can be OBs or they can be lean. Obesity seen about 50 to 60% of PCO patients. And in 30 to 40% of lean patients. The main problem in these patients is that there is insulin resistance. Insulin is a hormone which controls our glucose levels in the body. Because of resistance, glucose to main the glucose levels more and more insulin is secreted by the body. So because of these hyper insignia various problems are seen that is, as I said there is increased level of male type of hormones that is the cause of all the problems moreover these effects the hormone level at the level of priturity also that is also the brain level also. Because of that also there is an ovulation but that ovulation doesn’t take this time. So these patients develop small small cists over the, in the ovaries and they do now ovulate and they end up in infertility. it’s a infertility requires, if patient has many, prolonged ovulations that is they continue to have irregular periods of three months, four months they don’t bleed. One has to give them treatment. So that they can bleed at least four times in a year to prevent the cancer of the uterus later on in life. Similarly for hyperandrogynism, that is for acnes and hirsutism. Along with the medical treatment, along with the lifestyle changes and the diet modifications local treatment has to be given for hirsutism, various hormones are given. Where this problem they are very conscious about this. They have to be given hormonal treatment and till they maintain the lifestyle and reduce their weight, they need external treatment in the form of epilation or acne treatment in the form of applicatious along with they need certain type of hormones and when they become adults, in the adults infertility when they conceive in pregnancy also we have to monitor them for various complications as I said initially. They are more prompt to glucose, metabolism, and to prevent the insulin agents have to given.
