Sertraline is an ssri, it should be safe if your liver and kidney are healthy. Sertraline usual dose is around 50 mg. Usually antidepressants are taken long term as opposed to benzodiazepines that are short term. You should be fine, given you exercise daily, stay relaxed. There is no such thing as safest antidepressant, all antidepressants have their own side effects profile.
But you may try mirtazapine as it has least side effects.

Cortisol is rare reason for anxiety and depression. Common neurotransmitter implicated is serotonin and it is not simple to understand the interplay between hormones and neurotransmitters ( but basic explanation can be given to a lay person). It needs understanding of neuroanatomy, neurophysiology, neurochemistry and psychiatry, apart from general medical knowledge how these produce the symptoms and treatment.

Hello Lybrate user
medicine is not the only solution, you have to take therapy sessions as well with psychologist. As you said you have been taking medicines since long. This regional brain deterioration starts near the age of 30 and accelerates by age 50. Such deterioration can even start earlier for stressed-out adolescents who suffer from sexual exhaustion.
