Typically, a medium-sized hernia will not cause pain. In fact, hiatal hernia problems typically only present in hernias larger than 6 cm or 2.5 inches. So long as your hernia is smaller than 6 cm or 2.5 inches, it should be manageable with self-care and medication.

Sometimes an untreated hernia can lead to potentially serious complications. Your hernia may grow and cause more symptoms. It may also put too much pressure on nearby tissues, which can cause swelling and pain in the surrounding area. A portion of your intestine could also become trapped in the abdominal wall.

Surgery can repair a hiatal hernia by pulling your stomach back into the abdomen and making the opening in the diaphragm smaller. The procedure may also involve surgically reconstructing the esophageal sphincter or removing hernial sacs. However, not everyone who has a hiatal hernia needs surgery.

The only way to effectively treat a hernia is through surgical repair. However, whether or not you need surgery depends on the size of your hernia and the severity of your symptoms. Your doctor may want to simply monitor your hernia for possible complications.

A small hiatal hernia usually doesn't cause problems. You may never know you have one unless your doctor discovers it when checking for another condition. But a large hiatal hernia can allow food and acid to back up into your esophagus, leading to heartburn.
