Hello sir,
Sinus tachycardia may be due to various reasons, like anaemia, hypoxia, fever n also due to changes in heart functioning
R/S inversion may be due to panic attack also, if it's amplitude from V2-V4 is less than 5 mm then it's MI n if it's more than 5 mm then it's early repolarization
Better to get 2D Echo test done to rule out exact problem.

ECG i.e. elecro cardio gram shows electrical activity of the heart. If some is wrong with it like reduced blood supply(less of oxygen supply) it will show abnormal changes.
