Cortisol is rare reason for anxiety and depression. Common neurotransmitter implicated is serotonin and it is not simple to understand the interplay between hormones and neurotransmitters ( but basic explanation can be given to a lay person). It needs understanding of neuroanatomy, neurophysiology, neurochemistry and psychiatry, apart from general medical knowledge how these produce the symptoms and treatment.

medicine is not the only solution, you have to take therapy sessions as well with psychologist. As you said you have been taking medicines since long. Anxiety and depression are the result of stress of some past experience. Cognitive behavioral therapy is very helpful for this condition. Cbt addresses the here and now, and focuses less on the past. Along with medicines, talk therapy is required.
Only medicines will not solve depression. We need to identify the root cause of depression and solve it.
I recommend doing exercises and yoga daily.
Find good friends to share your feelings
keep your self engaged in studies/work/ any hobbies
