Hi, in the given context, where your psa level is 18.5 (i. E. Above 10), there is high possibility of you having prostate cancer. However, sometimes a benign enlargement of the prostate (bph) can also raise psa levels. To rule it out, ideally, a psa tumor marker test needs to be followed by a digital rectal exam (dre), and a biopsy to clinch the diagnosis. Biopsy is, indeed, the gold standard to diagnose prostate cancers, and the histopathological grading thereof. An mri scan can subsequently be done, the results of which along with the psa report helps stage the cancer accurately. Please proceed with the needful diagnosis and treatment, as deems appropriate, without delaying any further. Do take care and all the very best. Sincerely,
