Mr. Lybrate-user, diabetes mellitus occurs (t2) either due to lack of insulin or resistance to its action or both. In t1 there is total absence of insulin, beta cells of islets of langerhans in pancreas do not secrete any insulin. Since main reason is insulin deficiency, best drug for treatment is insulin. However, in t2 initially metformin is used, which helps in reducing resistance to insulin action and also helps in weight reduction. Then other drugs known as secretogogues (insulin secretion stimulators) are used. If diabetes is very severe with very high blood sugar levels and hba1c% >9, then insulin is preferred. So it depends on individual case and needs of each patient. There are multiple choices now, recent addition to treatment has been sglt-2 inhibitors, which act independent of insulin.
