Desmoid tumour or fibroma is a benign tumor, but with high chances of recurrence despite clear margins. The size of the mass, symptoms and respectability would determine the course of treatment. Surgery with clear margins is the main line of treatment. Reconstruction with primary closure or a flap if a large defect is advised. If slowly growing and asymptomatic desmoid, especially if requires major and difficult resection, then observation may be advised. Some times in such large tumors, where negative margins are difficult, some centres advise metronomic therapy for 6-12 months, with tamoxifen, methotrexate and Vinblastin, which helps in stabilising the disease and then operate. If Patient is unfit for surgery then radiation may be advised. Feel free to contact me directly if you want to share images or reports and discuss this further.
