I am boy aged 24 years, most of times I lost in my own thoughts and start talking to my self loudly, what should I do?

If you are aware that you speak to yourself loudly then that is not a problem. But if you are not then we have a concern and you should meet a counselor immediately. Anyway you will need to speak to a counselor to find out the nature of the stress you manifest. If that person feels that you need to meet a doctor please do so without hesitation. Even if you are put on medication do not discontinue the counseling which you will require for quite some time. It will help if you know your family history and your developmental background and furnish the same to the counselor to trace the origins of the stress. Are you aware of any fear that is overwhelming you? that could just be a simple beginning to get you started. Please cooperate promptly with the therapies because the prognosis will be very good if you start early before it gets chronic.
