A tumor, 19×17×16 mm sized, in bladder? At what stage and at what grade is it generally? I know cystoscopy is necessary. But in general, the situation how is going on?

One cannot assess the grade and stage of the tumor without a biopsy. The minimum you require is a TURBT. Whether it is muscle invasive or not will be clear with the HPE report of the specimen, the staging work up is usually done after TURBT.
