What You Can Expect After Undergoing Thoracic Ultrasound?

MBBS, MD - Pulmonary Medicine
Pulmonologist, Faridabad
What You Can Expect After Undergoing Thoracic Ultrasound?

When your doctor suspects of the presence of extra fluid in your lungs, you might be advised for a thoracic ultrasound. What is a thoracic ultrasound and why do you need it? With a thoracic ultrasound, your doctor will be able to look at everything in the thoracic cavity and can know if your lungs, heart, and other structures in the chest are working fine. The pulmonologists can efficiently determine the lung condition even in critically ill patients using a non-invasive ultrasound technique.

And, since the method is based on sound, there is no risk of exposure to potentially harmful ionizing radiation or nephron-toxic contrast dye in this case. Hence, a thoracic ultrasound has become the most preferred technique for conducting a preliminary examination or to further check and confirm a finding noted using other imaging techniques for thoracic diseases.

Reasons to have a chest ultrasound

  • The doctor usually refer an ultrasound when he or she can feel that there is some extra amount of fluid in your chest. The ultrasound can tell the physician the reason behind the deposition of excess liquid.
  • The ultrasound would help detect the type of fluid that is present in the chest, whether it is exudate that is caused due to inflammation, an infection, or lung cancer, or whether it is transudate that is a leakage from the lymph nodes or the blood vessel. It determines the movement of the diaphragm.
  • Chest ultrasound can be done in conjunction with other examinations like CT scan, MRI to evaluate the condition of the chest.
  • It has to be noted that, in certain circumstances such as severe obesity and barium in your esophagus, the procedure can give an inaccurate result. Hence, it is always advisable to talk with the health care provider and share your complete medical history before undergoing this procedure to prevent such occurrences.

After a Thoracic Ultrasound

  • After the results of the ultrasound are received, the doctor can detect what the condition of your chest is at the moment. Your doctor will also confirm if the lungs have collapsed or if there is water in the lungs, or excess fluid has deposited into the lungs.
  • Ultrasound can also reveal if you have pneumonia. A diagnosis like this does not take a long duration, and thus the ultrasound plays a significant role in expediting the treatment process.
  • There are no hard and fast regulations to follow after the ultrasound. The physician may recommend few instructions depending on your situation. In typical cases, there is no special preparation like fasting is required and you need not be sedated during the ultrasound as well.
  • A safe and painless method to detect the condition of the chest and the respiratory organs and the blood flow through the organs in your chest, a thoracic ultrasound is a preferred procedure that can help your doctor to diagnose your condition and decide upon the treatment method.

Vaginismus - Make Sex Less Painful!

Vaginismus - Make Sex Less Painful!

Sexual intercourse for humans is not only a reproductive act, but is also considered to be an emotional and pleasurable act. However, statistics report that almost 30% of the women experience pain during intercourse also known as vaginismus. This not only hampers the pleasure, but is also an unhealthy sign since it is indicative of a problem.

Vaginismus / Painful intercourse could be a regular occurrence, or a sporadic one. There could be numerous reasons that cause the discomfort and pain. Though it might be an early manifestation of grave diseases such as endometriosis or fibroids, but more often, it is triggered due to conditions such as vaginal dryness, yeast infection, bacterial vaginiosis, which can be easily treated. It is pertinent to understand that it is not the physical infirmity of the entire female anatomy that causes such pain/ discomfort, but the existence of certain conditions that can be treated by a medical practitioner.

Certain such causal factors associated with painful intercourse and the efficient ways to deal with them are discussed below:

  1. Vaginal dryness- Vaginal dryness is one of the most commonly cited causes for painful intercourse. There are various causal factors of vaginal dryness. These include, but are not limited to, consumption of certain drugs, bathing with hot water, health conditions or even lack of foreplay. Vaginal dryness due to the aforementioned causes is a short-lived condition that can be treated well with the help of a medical practitioner. Usage of lubricants that is suitable for you, as advised by a medical practitioner, could go a long way in aiding the ease of sexual intercourse. The lubricants act as an effective substitute, if the body is unable to naturally secrete vaginal fluids to ease intercourse, thereby greatly reducing the pain that is experienced.

  2. Improper vaginal expansion- The female anatomy is designed in such a way that it expands normally during intercourse. Proper foreplay enables the woman to get excited, which results in the release of vaginal fluids that aids in sexual intercourse. However, in the event that the female is not excited enough prior to the act, sexual intercourse can become a painful act, since the vagina is unable to expand in the absence of vaginal fluids. In such cases, take things slow and give your body time to comfortably go with the flow.

  3. Vaginal itching caused by infection- Vaginal infections can be caused by antibiotics, prior sexual activity that has resulted in contraction of Sexually Transmitted Diseases etc. This results in an itching or burning sensation while having sex. It is essential to rest your body instead of forcing things, and to consult your doctor as soon as possible. The doctor can correctly diagnose the causal factors behind the infection, and prescribe the necessary medicines for it.

  4. Emotional issues including fear of sex, marital issues/ disputes, etc are also leading cause of painful sex

However, symptoms vary from the common ones mentioned above, it is always advisable to consult a experienced doctor including a gynecologist, sexologist or sexual counselor.

Role of sexual counselor is very important, as he she can offer Sex therapy in the form of counseling, brief psychoanalysis, or behavioural therapy .This can help to address any underlying psychological issues, such as fear or anxiety, tackle any irrational beliefs that you have about sex, educate you about sex.

Also, some special techniques can be taught eventually stop your vagina closing involuntarily, such as using vaginal trainers and pelvic floor exercises

If you wish to discuss about this problem, you can consult a doctor and ask a question.

Rhinoplasty - Know All About It!

Cosmetic/Plastic Surgeon, Chennai
Rhinoplasty - Know All About It!

Rhinoplasty, more commonly known as nose surgery, is a cosmetic form of operation designed to change the shape and size of the nose.

A rhinoplasty can effectively change the size and width of the nose to complement the facial balance. It also helps improve nasal symmetry efficiently enhancing your facial features.

How is the surgery performed?
You can opt for either general anesthesia or local anesthesia for the surgery. The surgery is generally an outpatient procedure, but occasionally patients are asked to stay in the hospital overnight post the surgery.

The operation involves an incision which is made for easy access to the cartilage and bones of the nose. These incisions are generally placed inside the nose, so that they do not appear as scar marks after the surgery is done. Removal or replacement of the bone or cartilage depends on the demands of the restructuring of the nose. Once the reshaping of the nose is done, the nose tissue is put back into place and a splint is temporarily placed to support the new structure of the nose. The splint is removed once the nose heals.

Precautions to take after a nose surgery

After a nose surgery, the area around your nose will stay swollen and bruised for a considerable period of time. Thus you have to ensure that-

  1. You get ample amount of rest
  2. Use cold compresses over the swollen nose area, this will reduce the swelling and ease the pain
  3.  Do not indulge in relatively vigorous physical activity for sometime
  4.  Take you medicines regularly

Risks of nose surgery

Every surgery or medical procedure involves certain risks, and so does rhinoplasty. Along with some puffiness and bruising around the eye and nose region, you may also experience-

  1. Some amount of bleeding
  2. Injury to the septum area
  3. Irritation of the skin and skin problems
  4. Infections
  5. Nasal blockage due to swelling in the inner nasal area
  6. Complications resulting from anesthesia

Although there are certain risks to rhinoplasty, the surgery is considerably safe and the results are quite permanent.

CT Angiography Of Brain - Know Its Benefits!

MBBS, MS - General Surgery, Mch - Neuro Surgery

CT Angiography Of Brain - Know Its Benefits!

A brain computed tomography angiography (CTA) is a specialized neurologic CT scan, and it involves fast CT imaging while simultaneously injecting IV contrast into a vein in the arm. This procedure allows visualization of the specific vascular anatomy of the organs in the body. It can also be used to evaluate vessels or plan a surgery. Mostly it is used to identify a small aneurysm or arteriovenous malformation (abnormal communications between blood vessels) inside the brain.

  • A patient can expect the following things during the brain CTA examination:
  • The patient is asked to lie on the examination table.
  • If contrast is used, an automatic injection pump connected intravenously will release the contrast material at a controlled rate.
  • Although the movement of the table occurs slowly at first, it gains rapid momentum when the actual CTA is performed.
  • The patient may be asked to hold his/her breath during the scanning.
  • Any motion in the form of breathing or body movements can lead to artifacts on the images.

The entire CTA exam may be completed within a few seconds. However, the patient’s actual time in the room may be long. This delay can occur as the technologist takes his/her time to position the patient on the table, check or place an IV line, do preliminary imaging to verify the beginning and end points of the exam, and set up the scan and contrast injection settings based on the part of the body being imaged.

Benefits of brain CTA include the following:

  • The need for surgery may be eliminated by angiography; however if surgery remains necessary, it can be performed with much accuracy because of brain CTA findings. As the CTA is able to detect the obstruction of blood vessels, it allows for potentially corrective therapy.
  • The CTA may give precise anatomic detail than a magnetic resonance imaging (MRI), particularly in cases of small blood vessels.
  • The CTA is faster, noninvasive, and has lesser complications than catheter angiography, which involves placing a catheter (plastic tube), (usually at the groin) into the patients’ major blood vessels and injecting a contrast material along with probable sedation or general anesthesia.
  • The CTA examination costs lesser than catheter angiography. No radiation remains in a patient's body after a CTA examination.

Risk factors for brain CTA

A small chance of cancer from excessive exposure to radiation History of allergy to X-ray contrast material Risk for kidney failure, as the contrast material could potentially further damage the kidney function in patients with pre-existing kidney disease

Limited Alcohol Consumption - Can It Be Beneficial?

Limited Alcohol Consumption - Can It Be Beneficial?

It is common belief that alcohol consumption is a social menace and is injurious to health. It is the third leading lifestyle causes of death in the United States and takes a toll on the health care costs of the country. Many organs like the liver, kidney, heart, and brain are affected in people who are addicted to alcohol. However, it has been proven that alcohol consumption in minor amounts can have beneficial effects on the human body including the heart.

The side effects depend on a number of factors listed below.

  • Type of alcohol consumption (social or habitual drinker)
  • Amount of alcohol consumed
  • Frequency of drinking
  • Age, gender, and genetic predisposition of the person to develop alcohol-related diseases
  • Family history of alcoholism and its related diseases
  • Age at which the person started drinking
  • The number of years that a person has been consuming alcohol
  • Overall health condition of the person
  • Exposure to alcohol as a fetus

Like the adage goes, anything in excess is bad. It is not necessary that a habitual or social drinker who consumes a drink or two per week would end up with these issues. In fact, if recent studies are to be believed, mild to moderate intake of alcohol does have a benefit to overall health.

  • The cardiovascular benefits are the most prominent. Red wine and beer, in particular, are shown to provide benefits against cardiovascular damage in the long run. This could be due to the antioxidant properties of red wine. Alcohol itself (ethanol or ethyl alcohol) also has a positive effect. Some varieties of beer, porter and stout varieties in particular, are also shown to be beneficial. The anti-inflammatory properties and antiatherogenic (effect on plaque formation in the blood vessel) are what contribute to these positive effects.
  • There is evidence to show that cholesterol profile is improved (good cholesterol increase) and clotting function is improved in people who have mild alcohol consumption.
  • The insulin sensitivity is also better, leading to better control of sugar levels.
  • Light alcohol consumption also is believed to reduce the risk of hemorrhagic stroke. However, heavy alcohol consumption is believed to increase the risk of stroke and other cerebral events.
  • Limited use of alcohol is also linked to reduced risk of dementia. Heavy consumption leads to faster memory decline.
  • The key to note is that these benefits are only when the alcohol consumption is mild to moderate. In excess, the negative effects of alcohol are well established and numerous.

Some of the most significant ones being cirrhosis of the liver and cancers of various organs including mouth, liver, larynx, oesophagus, colon, breast, pancreas, etc. To reduce these effects, alcohol screening and brief counselling to reduce habituation are helpful.
