I have a some compression between L4 and L5 as per MRI for the last about 5 years. During walk (occasionally) I feel some difficulty in walking and take rest or I need to sit for about 5 minutes and than feel normal. Please advice if there is any medicine or physiotherapy ? Pramod

DNB, MBBS
Orthopedist, Delhi
Pain in the leg can be due to nerve compression in the back or inadequate blood supply to leg due to vessel compression or narrowing of arteries. Your MRI will have to be seen to evaluate amount of compression. If compression is mild, then an arterial and venous Doppler of both lower limbs will help to assess the blood supply to the legs. The treatment depends on the cause of the pain.

I have back pain when I'm continue working for more than 1 hour so what should I do.

I have back pain when I'm continue working for more than 1 hour so what should I do.
Back pain can result from bad posture, a sudden jerky movement, a lumpy mattress or poor lifting techniques. It can also be caused by injury in a work place, by a sports accident or by muscular spasms. Or, because of decreased flexibility, as people get older. There are also many diseases and pathological conditions that can lead to back pain. These include abdominal or pelvic disease, anxiety, arthritis, cervical or lumbar spondylosis, dermatological problems, kidney disease, rheumatic conditions, tumours and scoliosis. Our modern, sedentary lifestyles have a profound effect on the development of back pain; indeed one of the most effective ways of preventing it is simply to stay active. An average adult in the India spends at least two hours a day in front of a computer screen or television set, and back problems can be triggered if they don’t sit properly. In an age of mobile phones and computer games, such troubles are increasingly inflicting children of school age as well. Top tips for back care: For back pain, better to see your Doctor sooner than later. Take regular exercise – your Doctor can say what’s right for you. Hours in one position can cause problems – avoid computer ‘hump’. During repetitive tasks, vary your rhythm and take frequent breaks. Adjust car seats and on long journeys, take regular breaks to stretch. Pace yourself with heavy work such as gardening – don’t risk a disc! Watch children’s posture – they shouldn’t carry bags on one shoulder or spend too long at a computer without breaks. Avoid strain when lifting, particularly small children and shopping. Your bed could be part of the problem. Take a hot bath or shower, soaking in a hot tub, or using a heating pad, hot water bottle, or heat wrap that provides continuous, low-level heat are all ways to bring healing warmth to your lower back.

Chickenpox needs to managed according to the symptoms shown, if you have fever then take medicine for fever as prescribed by doctor, as a protective measure, it is required to stay at home while they are infectious to avoid spreading the disease to others. Cutting the nails short or wearing gloves may prevent scratching and minimize the risk of secondary infections. Being also a general physician and registered dietitian, I prescribe both evidence based herbal as well as allopathic medicines complementing with dietary guidelines and home remedies carefully personalized for each individual patient.

DNB, MBBS
Orthopedist, Delhi
Sir, there is no cure for ankylosing spondylitis in allopathy, drugs available are only to control or delay the disease process, so drugs will continue lifelong. Side effects and efficacy of the drugs will have to be monitored from time to time and doses adjusted accordingly.

Calf pain after an injury is usual but runners often complain of calf soreness with no history of trauma to the area. In these cases, like many in running, the key is identifying the cause and rectifying it. Non-traumatic calf pain usually follows a fairly predictable pattern – the pain develops when running and gradually worsens as the run continues. The calf may feel tight and even stop the runner from going any further. After running the pain subsides a little but the calf often continues to feel tight for a day or so. Commonly when not running symptoms are minimal. This leads to the question of why are my calf muscles becoming fatigued? Every muscle has it’s different level of strength and endurance, exceed that level and it will usually start to become painful and tight. The answer to why this happens usually has 2 parts to it; 1. The calf is being overloaded 2. The calf muscles are weak or lack endurance. Overloading the calf Common causes can be introducing hill or speed work, increasing weekly mileage and increasing training intensity .Another factor is exercise you do in addition to running – if you’ve started to introduce gym sessions and running on the same day, or the following day, the calf may already be somewhat fatigued before you start. If you run regularly you might be quite accustomed to running on heavy tired legs. You can become unaware just how fatigued muscles are getting. The first answer when you have been overloading your calf is often rest. A few days rest, some will often work wonders. I would recommend this before you start thinking about addressing any calf weakness – adding more exercises to an already fatigued calf can add to the problem. Secondly, you may need to think about your training schedule. Are you doing too much with too little rest? Adding a rest day or reducing your mileage temporarily can help resolve symptoms, allowing you to progress again Assessing calf strength The easiest way to do this is a single calf raise; • Stand on 1 leg with your finger tips on a wall/ table for balance (not to push up from). • Push up onto your toes and slowly down again • Do as many as you can (going right up, not just lifting your heel a bit!) • Count the repetitions and compared left and right side You should be able to do the same amount left and right and it should feel as easy both sides. Clinically I like to see runners achieving 40+ reps on each leg. Below 30 might suggest a lack of endurance. You may find this test causes your symptoms, in which case stop, don’t push through pain. The single calf raise is very effective to strengthen the calf. Do as many as comfortable, rest for 1-2 minutes and repeat for 2-3 sets. Aim to work up to 3 sets of 25-30 reps. Do this 2 or 3 days a week on days that you aren’t running. The exercise can also be done on the edge of a stair to allow for greater range of movement by letting the heel drop below the level of the step. Calf flexibility is also important and not to be overlooked. Gently stretch the calf dynamically prior to running using mini squats, lunges, wall presses etc. anything that stretches the calf a little in a comfortable, controlled way. Dehyration aspect cannot be ruled out.Runners who drink less water or sweat more ,often suffer from Calf cramps. So one should drink large quantity of water if they run in routine. Homoeopathic medicine RHUS TOX 30 ( Dr Reckeweg) Drink 2 drops 3 times daily for 7-15 days.
