For reducing wt follow three main things like healthy diet, exercise and brisk walk morning and evening which increase your metabolism. Take high fiber food in your diet. The symptoms you have described can be due to fluid retention in the body which can have cardiogenic or nephrogenic causes, consult a qualified cardiologist and get appropriate investigations done i.e. 2d echo, treadmill stress test, cardiac enzymes and get the medications modified accordingly.

Please maintain the chart of BP and communicate with her doctor. Regular personal check up ( personal checking of all the vital parameters is important ) and some investigations would be helpful to know the reason. At the same time let her be disciplined about her lifestyle ( daily routine and the healthy diet ).
