As you said your physical activity is nil. Try to improve that first. Plus you need to control your stress and anxiety. Healthy diet low in salt, fat, simple carbohydrate and rich in fibres, vegetables along with daily exercise may help to control your bp.

1.Your blood sugar is under control, continue with metformin tablet
2.Your Bp is not under control. The combination of amlodepine with hydrochlorothiazide is effective in controlling Bp.
3.Continue for some days, reduce your salt intake, control your diet, go for regular exercise.
4.Monitor your BP and if still it is not controlled, consult physician for alternative medicine like Telmasatan with amlodepine

This high blood pressure may be the cause of his headache & uneasiness. This BP needs to be brought down immediately so It will be best to take him to a local hospital and get BP lowering treatment started under the observation of a cardiologist, if there are no cardiologist near you, then consult an MD medicine.
Once the BP stabilizes take him to a cardiologist for a detailed cardiac evaluation.
