First you need to give him a glycerine enema like neotonic enema. Then start syp loose/duphalac 15 ml at bedtime & continue for atleast 2 months. It's very important to continue the meds for 2 months or more otherwise the symptoms will return. Also, toilet training should be started & child should be given plenty of water & fibre rich diet.

Hello Mr. Lybrate-User,
perianal itching is a common problem. It is very annoying and often embarrassing too. The common causes of itching around the anal region are piles, fungal infection and intestinal worms. Hence kindly do not worry, it is not due to any major problems like cancer, inflammatory bowel disease or tuberculosis.
You have mentioned that piles problem has been cured. So you need to cure fungal infection and worn infestation. You can start. 1. Tab zoocon once daily for 5 d...more

Please get yourself examined by a surgeon. It may not be possible to avoid surgery for a fistula in ano. Get an MRI fisulogram done to visualize the entire fisula tract. You are likely to require Fistulectomy.

Hi Mr. lybrate-user, the medications you were prescribed are meant to control your symptoms. But the primary cause if any anorectal disease is mainly constipation followed by straining. So any thing you apply, problem may come back if you can't remove the causes. If the problem becomes too strong to interfere in your normal life style, then it is the time to ask about surgical options which will definitely give you long term and sustainable results. Thanks and regards.
