The blood glucose levels mentioned are high, suggesting there is no control on glucose levels. Ideally fasting glucose should be < 100 mg, PP 150 to 160 mg & HbA1c% < 6.8 (not mentioned). This shows clearly that your present treatment is not adequate. There has to be an up-gradation of treatment, either by increasing the Insulin dosage or adding some other oral drug. There is no fixed schedule for taking anti-diabetes medications. Schedule can be altered based on the need of a specific patient. Most important is the blood glucose control achieved by the patient. Ideally with treatment fasting blood glucose should be < 100 mg, Pp 160 to 170 mg & HbA1c% < 7. With HbA1c% 10, it is clear that diabetes is not in control. That is the reason Insulin injections have been added in the treatment. There is no mention as to from when is on the insulin injections. Plus it is also necessary to know her serum creatinine & urea levels, to look at the possibility of reducing or discontinuing Insulin. Unless blood glucose level is strictly controlled, it is not possible to discontinue Insulin. Thanks.
