I have seen the details given. From that I learn that you are not having adequate control on blood glucose and high bp. However, there is no mention about tsh values (to know whether hypothyroidism is in control or not), as well as lipid profile.

Yes it is time that you should be concerned about what is the actual problem that is causing all these symptoms for your mom. These symptoms can be either due to heart or kidney, but for that we need to get her investigated. You should get the following tests done - RFT, Lipid Profle, LFT, ECG and a good Echo and the consult a cardiologist near you or you can take her to cardiologist and then get a proper evaluation done.

Few diet tips - Not to eat fried food items and Can eat All green vegetables, moong , chana , Dal , rice etc And to apply ghee over chappati and don’t apply oil over chappati And to drink milk mixed with turmeric powder twice a day And If possible eat eggs on alternate days And If possible in breakfast eat atleast one bajra ka rotla And Eat seasonal fruit

2. Reduce body wt 3. No smoking/ tobacco 4. Diet - no ghee/ butter, have mix of vegetable oils - mustard, til, ground nut, olive oil, have more green vegetables and fruits, have whole grain atta, no fried. Fast. Spicy / processed/ junk food. No sugar, potato, rice
5. 30 mts brisk walk daily 6. Deep breathing exercise for 10 mts daily
7. Meditation daily for 10 mts ,6-8 hrs of sleep at night
