Hi, My father is suffering from systolic hypertension. He is diagnosed 3 years back. The hypertension history is first he was given with amlodipine+valsartan 5/80 mg (exforge) before sleep when he take that medicine his systolic blood pressure become normal but diastolic decreases to 50 or less then the medication was changed and he started with amlodipine 5 mg (Norvasc) before sleep but the problem remains same then the physician changed it from 5 mg to Amlodipine 2.5 mg (Lodopin) before sleep but the problem remain same and there was no success. We were referred to cardiologist who after taking the echo and ecg said that he is having no cardiac issues and suggested losartan 50 mg (eziday) before sleep. Now with this medication the issue is that his diastolic comes to normal if he takes medicine but systolic is going high up to 175 - 185 then he changes it to valsartan 80 mg (diovan) before sleep but the problem remain same with this medicine but the slight change that appears is his blood pressure remains normal till mid day and after mid day it starts going up and till night it again reaches to 165-175. I want to ask is there any solution for the above case. Your response will be much appreciated.

Go back to amlodipine. Keep systolic BP less than 150. Ignore diastolic BP. Measure his BP only once every two weeks on fixed dates after resting for ten minutes. Completely avoid measuring at any other time.

Dear Doctor,   Good Day! I am seeking treatment for my mother Age: 71, a widowed housewife. My father died before 30 years ago when I was only 3 years old, after that she got Diabetes. I think anxiety and depression regarding family maintaining and raising us for the past 30 years. I have found you very good doctor in Lybrate and other medical related apps. My mother’s condition is as described below:   Current Complains: - Recurrent foot infections (After using antibacterial ointments Mupirocin, Dermavet etc.) - Dry foot - Pain, Burning and tingling sensation all over the body - Insomnia - Palpitations     Chronic: Hypertension (avg 90/140) Diabetes (for 30 yrs) Nerve/Neuropathy pain Lower back pain / joint pain Osteoporosis  Acidity and Digestion disorder  Constipation  Insomnia urine obstruction / Urin amount decrease often (Recurrently occurs)   Current Medications: 1.       Mixtard 30/70 (22 + 10 + 12 units) 2.       Ecosprin 75 (0+ 1 + 1) 3.       Metoprolol tartrate 50 mg (1+0+1) 4.       Amlodipine 5 mg (1+0+0) 5.       Losartan Potassium 50 mg (0+0+1) 6.       Trimetazidine Mr. 35 mg (1+0+1) 7.       Esomeprazole (1+0+1) 8.       Calcium 500 and Vitamin D 200 IU (0+0+1) 9.       Bromazepam 3 mg (takes in case of sleeplessness) 10.    Neuro-B Thiamine Monotritrate, Pyridoxine HCL, cyanocobalamin (1+0+0)   Now, I am seeking your kind suggestion over online. It will be a great help for me if you give me a proper treatment plan and I have also a plan to visit Kolkata within 10 days and will stay there for 3 to 4 days.

The nerve pain is due to long standing diabetes. That is what will lead to recurrent ulcers on feet. Please moisturise feet daily. Take treatment immediately if there is any sign of infection. Control diabetes and blood pressure well. Do not let her walk barefoot

Telmisartan plus Amlodepine (TELMA AM) is a good and safe combination. The other tablet mentioned is Losartan + Hydrochlothiazide. That too is good. Doctor must have changed to newer drug for getting better control. But if she is not comfortable with that, she can take the earlier combination. Thanks.

Losartan 25 x 2 is very similar to 50
ecospirin AV has two medicines, and side effects are possible. Plus 4-5 heart failure medicines. Check why these two are needed, or discuss case on phone. Best wishes.

It is one of the angiotensin receptor blocker used to treat hypertension. Other medicines of this group are telmisartan, olmisartan,irbesartan, candesartan,losartan etc. You can replace valent with any of these after consulting your doctor.
