Metformin is a safe drug to be used in diabetes because chances of hypoglycemia are very less with it.
Pioz (Pioglitazone) is another drug used to control blood sugar with different mechanism.
Usually second drug is added when blood sugar is not controlled with diet restrictions, regular exercise and single medicine.

I have gone through the details given, however, there is no mention of actual glucose levels like - fasting & PP and HbA1c%. If a person has Type! diabetes then only Insulin injection is given. Oral drugs do not work in that condition. Since you are also using Pioglitazone, that means it is Type 2 diabetes. Insulin is given to achieve a quicker control on blood glucose.
