Your ACR seems fine. Please mention your serum creatinine (mg/100 ml) and urea concentration to calculate eGFR. Also mention the fasting, PP glucose levels and HbA1c%. Plus give following details : dietary pattern, extent of daily exercise being done, lipid profile, weight and height. On going through all the parameters I will be able to give specific comments. Thanks.

Get a Holter monitoring test done which records you each every heart beat for continuous 24 hours. But consult a Cardiologist preferably.

However if you have provided complete information and there is no other reason, you have an element of diabetic neuropathy developing in your case.
Now, the full workup would include a detailed clinical history of yours, in order to rule out any other cause of your symptoms so as to proceed with the treatment.
