First of all, there is nothing like central diabetes mellitus.
Sometimes, excess gh or acth release from pituitary from the tumor may cause hyperglycemia called as secondary diabetes.
Low secretion of adh may cause excessive urination known as diabetes insipidus lead to excessive thirst. But sugar remains normal in the blood. So please be more specific about his illness and give a proper history of what hormones he is taking regularly.

If it has had no effect after a few days, it is unlikely to work at all. However, sometimes the initial dose is not high enough. A doctor may advise that the dose be increased if it does not work at first. Also, it is possible that food can affect the absorption of desmopressin tablets into the body.
