The use of lantus can be combined with either a short acting or intermediate acting insulin depending upon the sugar values and its your doctor judgement to decide the dosage because short actings effects will last for three hours and intermediate effects will last for eight hours.

Hello, Thanks for the query.
Both Insulins mentioned are same. Only difference is one has 40 units per ml in the vial. The other has 100 units per ml in the pen. So if you are taking 6 units with 40 u/ml one, then 6 units need to be taken with the pen. You will have to fix the dosage on the pen, it will deliver that quantity only. Thanks.

In long standing diabetes pt whose beta cells are exhausted and who is obese insulin requirement may go beyond 2-3 units/kg. HbA1c is approx average of blood sugar control in last 3 months. More HbA1c means more uncontrolled diabetes and may require more insulin.
