I am male 46 years old suffering from major depressive since last 22 years. The first episode was very very severe with severe numbness and sleepy throughout the day, complete impairment in memory, thinking n thought processing tried different kinds of depression medications flunil 40, sertraline,veniz 40 etc, even though flunil helped me to some extent but my psychosomatic symptoms like body pains constant fatigue never gone away and also memory impairment n executive n thought processing capabilities not improved much even after medication for 22 years. Cognition n memory impairment was my biggest problem. One year back lamictal 200 introduced along with antidepressant lamictal helped me to a great deal in restoring my subnormal n below baselevel depressive mood lifted to certain extent far better than antidepressant alone. My main concern now is improvement in mood to further extent by using another suitable mood stabilizer along with lamictal n my grave concern of cognition and memory improvement which never returned back .I used to be with excellent n sharp memory and was topper through out my studies until this depression episode occurred. Please suggest add on medication for moodlift and memory problems.

I can understand the problem you are facing from such a long time. 22 year of long illness without recovery even when you are compliant with medication. What I suggest is to rule out other possibility causing depression such as thyroid or having bipolar mood disorder. Sometimes we miss out hypomania episode as depression covers it. I have been suffering from ocd, depression and insomnia since when I was 8 or 10 years old. I am taking treatment for it since 8 years after consulting with psychiatrist. Since last 1 year I am taking fluoxetine and zolpidem 10. I tried many medication but not helped for ocd and after taking fluoxetine 40 mg obsessive thought and compulsive behaviour which was so debialating is almost recover, you can say 99%. But insomnia is not going to heal. Whenever I took zolpidem that night only I could sleep. And I felt high when I use to take zolpidem and use to take 2 to 3 tablet instead of 1. So, I was worried that I got addicted to it and switched to new e-consultation doctor from docsapp. She was psychiatrist and ahe had mbbs and md degree. She prescribed me fluoxetine 40 and clonazepam 0.25 mg at morning and lamotrigine od 50, clonazepam 1 mg and mirtazapine 15 mg at night. Now my sleeping problem is resolved. Is this right treatment? Can I trust doctor from internet of docaspp? She added lamotrigine od 50 mg because I told her that only night time after reaching home from duty I feel very depressed so she told me that it is mood fluctuating anf lamotrigine will help. Should I trust her because she is from docsapp and I had not saw her face to face. Please help.

We understand that you have been diagnosed with schizoaffective disorder and it is not unusual that people with this problem take medications for a very long time, probably for a lifetime. However, like your psychiatrist has rightly advised you might want to decrease the dose to find out how you function on a daily basis while being on the lower dose before going further. Paroxetine belongs to ssri group of medications which though are good antidepressants cause sexual dysfunction commonly. But not to worry, they are reversible and you need to consult your psychiatrist and get your prescription revised for control of your depression as well as improvement in your sexual performance do not take these on your own for many years, then the problem becomes much more difficult to handle. such parasomnias are common in depression. It might be restless leg syndrome also. Unless I know the diagnosis, I can not give add on medication. Please undergo sleep studies overnight with all your routine tablets and discuss with report.
