It may be a ringworm infection.
Ringworm requires proper treatment and supervision, as it is contagious and may spread the whole body. A general or normal ointment may not assure/cure it completely, though for time being it disappears it appears in another part of the body. Proper internal medication is a must.

You may be suffering from tinea cruris which is fungal infection of the inner thighs, groin and buttocks area. It is also called jock itch. Good afternoon. Clobetasol should not be used for ringworm. Your treatment needs to be amended. Kindly consult a qualified dermatologist either in person or via online consultation.
Ringworm is not related to any particular food. Have a well balanced diet. Avoid wearing wet clothes and while on treatment avoid sharing clothes and wash your clothes separately. Wear cotton clothing and avoid tight clothes and synthetic material.

As you mentioned yor age is 82, you are probably more free .dry skin is a common problem of old age which may be responsible for itching. As you are having this problem since 3 decades it may be a manifestation of an obsessive disorder.
To get rid of the problem, take a openion from another skin specialist. Do not think of itching always. Keep your self busy and occupied. Keep your nails trimmed. Apply vaseline on affected part. If this dose not help take openion of a psychiatrist.
