You have not mentioned your blood sugar fasting, pp and hba1c report.
It is better to start with first one as it contains two medicines and second contsins three. Monitor blood sugar and keep in touch with your doctor.

Hello sir. Do walk daily. Don't skip your breakfast ever. Have small and frequent meals. Avoid sugar, sweet, honey, white rice, maida produts, pumpkin, glucose, juice, excess salt. Mango, banana, leachi, chhiku, . Take plenty of water. Take high fiber diet . Take plenty of fruits.

It appears you are on a fairly high dose of Glimepiride. Metformin (2000 mg) is also fairly high. Usually Glimepiride should not be used more than 4 mg in a day. Even though recommended upper limit is 8 mg, conventionally using more than 4 mg is not advised. Since there is no mention of actual glucose levels (fasting, PP and HbA1c%), there is no way to know how good is the control. For your father check his blood for HBA1c and monitor his sugar levels with a glucometer regularly and if need arises decrease the doses.

You are suffering from Type 2 Diabetes.
