Hi lybrate-user, I understand that you are going through a depressive mood state. Duloxetine (duvanta) do works to alleviate depression but the dose at which you are taking it is not gonna help. In my opinion you need to increase the dose to 40 mg instantaneously and further increase to 60 mg per day in divided doses after next 3 days. Lets see how it goes and please let me know about progress after 2 weeks.

citalopram (Celexa)
duloxetine (Cymbalta)
escitalopram (Lexapro)
paroxetine (Paxil and Paxil CR)

Thank you for the query.
As per the symptoms you have provided, your wife is probably suffering from a condition called diabetic neuropathy. It is a condition which causes nerve damage in diabetics. High blood sugar can injure nerve fibers throughout the body, but diabetic neuropathy commonly damages nerves in the legs and feet. This leads to tingling/numbness, burning sensation and pain in the feet and hands (gloves and stockings distribution). A nerve conduction velocity test confirms the ...more

Medications can help reduce the pain of fibromyalgia and improve sleep. Common choices include:
•pain relievers. Over-the-counter pain relievers such as acetaminophen (tylenol, others), ibuprofen (advil, motrin ib, others) or naproxen sodium (aleve, others) may be helpful. Your doctor might suggest a prescription pain reliever such as tramadol (ultram). Narcotics are not advised, because they can lead to dependence and may even worsen the pain over time.

In case you have more symptoms like
1. Generalised bodyache
2. Fatigue
3. Lethargy
4. Unrefreshed sleep
5. Poor concentration
you might be suffering from a syndrome of fibromyalgia. You can google it out. It is a chronic pain sensitisation syndrome where all investigation reports are normal and is best treated by graded exercises, deep breathing and drugs like duloxetine and pregabalin.

1. Fibromyalgia is a diagnosis of exclusion, and difficult to treat with just medications. 2. If you explain your symptoms better with prior tests done, doctors here can take a look.
