My wife aged 38, weight 60 kg, height 5' 2" feeling burning sensation in her front part of her tongue since 2 month, no such injury seen on tongue. Took homeo treatment 1 month, got relieved for some time but again restarted. A doctor suggested B complex capsule, took for 1 month but no relief. Kindly suggest why this happens? Is there any reason for detail check up? Suggest medicine too, if required.

You can start with multivitamins and antioxidants after visiting someone who is in oral medicine practice. Visit a dental hospital nearby and get to oral medicine department along with patient and let them help you with it.

Each of the doctor here has given perfect reply to your question. Do follow them. OK what I will add here is you apply some icepack from outside and do lukewarm saltwater gargle at least 6-7 times a day. Hope this additional info helps too.

A problem pertaining to the storage function of the bladder that results in bouts of sudden, often uncontrollable urge to urinate is referred to as an overactive bladder. This condition which is marked by unconditioned or involuntary loss of urine can sometimes be quite difficult to stop. People who experience such a condition often feel humiliated and as such tend to limit their social and work life. Despite such, only a few are conscious that a brief evaluation can help them manage and overcome an overactive bladder.

Mechanism of Urination

During urination, the urine proceeds from the bladder and flows into the urethra which is located at the tip of the penis in men and above the vagina in women. As the bladder fills, the nerve signals in the brain prompts urination by coordinating the relaxation and contraction of the urinary sphincter muscles.

Causes and Symptoms of an Overactive Bladder

Primarily caused due to involuntary contraction and relaxation of sphincter muscles, several conditions can lead to overactive bladder. 

Some of them are: 
1. Parkinson's disease, Alzheimer's and other neurological disorders
2. Poor kidney function due to diabetes
3. Medications that lead to increased production of urine
4. Bladder abnormalities like tumors or stones

5. Constipation
6. Excessive consumption of caffeine or alcohol

Some of the common signs of an overactive bladder are:
1. Bouts of sudden, uncontrollable urge to urinate
2. Awakening at night frequently to urinate
3. Urinating more than eight times a day

Risks
The risk of an overactive bladder gradually increases with age. Conditions, such as diabetes and an enlarged prostate results in the increased likelihood of an overactive bladder. People who have previously faced strokes and heart attacks experience cognitive decline, which often times lead to the development of an overactive bladder.

Urinary incontinence as well as a host of associated factors can be detrimental to your life. Emotional distress, interrupted sleep cycles and depression are some of the observed complications of this condition.

Thus if you experience or entertain suspicion of an overactive bladder, you should consider visiting a general physician who might refer you to a specialist, if need be.

Bladder Cancer: Know Its Symptoms & Treatment

Bladder cancer can be caused by a variety of factors which include genetic factors, inheritance, drugs, chemicals, environment-related factors and smoking. In fact, smoking tobacco is one of the major causes of bladder cancer around the world. The chemicals present in tobacco can irritate the bladder lining which ultimately leads to bladder cancer. Besides smoking other important environmental factors might be chemotherapy, exposure to chemicals from newspaper, dye, leather, paints industries.

Symptoms of bladder cancer are as follows:

Most often it occurs in people more than 50 years of age. It can present in different ways but common symptoms being :

  1. Painless hematuria - reddish or brownish colored urine with or without clots Increased frequency of urination Pain while passing urine .
  2. Difficulty in passing urine

Diagnosis:

The diagnosis requires a proper imaging workup including ultrasound, CT scan and urine examinations. Following the initial imaging, a biopsy is required to confirm the diagnosis and stage the disease. This is done by cystoscopy under anesthesia.

Treatment Options:

Treatments for bladder cancer depend on the stage of cancer. Broadly bladder cancers can be categorised into Non-invasive and Invasive cancers.

  1. For non-invasive cancers: It requires recurrent cystoscopic surveillance and resection. For those with high risk of tumor recurrence or progression, intravesical chemotherapy or immunotherapy might be required.
  2. For invasive cancers the various treatment choices are:
    1. Surgery: For localised tumor which are amenable to resection perhaps surgery is the best possible treatment. This is a major undertaking where in whole of bladder along with accessory organs is removed. The surgeon may offer choices for reconstruction between neobladder (refashioning bladder with patient's gut) or ileal conduit (fashioning a stoma over which bag can be worn to collect urine). Following surgery a regular follow up is required and the prognosis is good.
    2. Chemotherapy: This modality is reserved for patient's where surgery is not possible or they have metastatic disease (disease spread to other body parts).In this method, medications are used to target and destroy cancer cells.
    3. Radiation Therapy: This might be and option for patient's that are not suitable for surgery. Radiation therapy uses gamma rays to destroy cancer cells in the body The key to good prognosis in bladder cancers is early detection timely intervention and good follow up.

The prostate gland is a male reproductive organ at the base of the urinary bladder and urethra is a thin tube that carries urine out of the penis. It runs through the prostate gland. A fluid produced by this gland helps to carry sperms produced by the testes outside during intercourse.

Cause of urinary problems in men

Men experience urinary symptoms either due to inflammation of the prostate gland or prostatitis or symptoms may be the result of a blockage of the passage due to enlargement of the prostate gland.

Symptoms of urinary problems

Urinary symptoms commonly experienced with prostate problems include;

  1. The need to urinate frequently during the night.
  2. Urinating more often during the day.
  3. Urinary urgency is the urge to urinate can be so strong and sudden that one may not reach the toilet in time.
  4. The urine stream is slow to start.
  5. Urine dribbling for some time after finishing urination.
  6. A sensation that the bladder is not fully emptied after urination.
  7. A lack of force to the urine flow, which makes directing the stream difficult.
  8. The sensation of needing to go again soon after urinating

Treatment

  1. A long course of antibacterial medication (for bacterial prostatitis) is prescribed, because infection is difficult to get rid of, the antibacterial medication will need to be taken for many weeks.
  2. Medication to improve urine flow and other symptoms (for obstruction caused by an enlarged prostate).
  3. Surgical procedures (for blockage caused by an enlarged prostate), such as Transurethral Resection of the prostate(TURP), Laser Resection of prostate, Transurethral Incision of the prostate(TUIP).

A number of other procedures that have been developed to reduce urinary symptoms.

Medication for urinary problems

Various medications to help ease your urinary problems, including;

  1. Medications to reduce the tone of the muscles of the urethra and prostate to minimize any obstruction to urine flow caused when these muscles contract.
  2. Medication to reduce the size of the prostate gland. These drugs work by blocking the action of male hormones produced by the prostate gland.
  3. Medications to relax the bladder, making unwanted contractions less likely and reducing the symptoms of urgency and frequency of urination.
  4. Over-the-counter medicine 'Saw Palmetto' (Serenoa repens) is used sometimes. This may help some men, especially if frequent urination at night is a problem. However, recent reviews of the evidence for using Saw Palmetto as a treatment for mild or moderate urinary symptoms did not show any improvement compared to no treatment, in men with BPH.

The pair of kidneys that human beings have, forms an important part in the excretory system. They perform specific and important functions. They excrete urine as well as help in eliminating the toxins, which accumulate in the body. Kidney stones occur when the minerals present in the urine without being discharged, keep on accumulating. This gives rise to an extremely painful and uncomfortable situation. Though sometimes the situation gets revoked all by itself, many-a-times it requires medical intervention. 

Some of the symptoms of kidney stones are

  1. Excruciating pain in the back
  2. Pain while urinating
  3. Tendencies to vomit
  4. Dizziness

However, there are several preventive measures which you can undertake in order to avert the situation. Some of them are:

  1. Drink lots of water: Kidney stone is one of those rare conditions that once occurred, always carry the latent threat of recurring again. Therefore, drinking water in large quantities becomes essential. The more you drink water, the more frequently you would urinate, thereby curbing the chances of the minerals settling in the kidneys.
  2. Avoid certain foods: Food like beets, spinach, those which are replete with calcium oxalate should be avoided in order to minimize chances of having kidney stones.
  3. Change your diet: If you are trying to avoid the eventuality of developing kidney stones, it is imperative that you change your diet a bit. It is recommended that you exclude animal protein as much as possible and assiduously avoid the intake of salt as much as possible.
  4. Take less calcium supplements: Calcium generally is considered a nemesis to your kidneys and is prescribed by doctors to be avoided. However recent research has thwarted this claim. Calcium does not pose a threat to kidneys. Only men have prostate gland and it surrounds that part of the tube which carries urine out of the body from the bladder. When cells in this gland start growing without any control and get clumped with each other, they form tumors. It is then the chances of prostate cancer arise in the men's body. These tumors can be malignant, and they can kill the healthy tissues of your body and spread to other body parts. 

Chances of getting prostate cancer
The chances of getting prostate cancer are among men who are over 55 years of age. The ratio of getting prostate cancer is one among six men. It is more common in black men compared to white men, Hispanic men and Asian men. Moreover, when there is history of prostate cancer in your family, the chances of having prostate cancer increases.

Now, let's find out the symptoms that will let you know that you may have prostate cancer.

Symptoms of prostate cancer

There are different stages of prostate cancer and depending upon the stage you can get to know various symptoms. 

However, the problem is that in the early stages there are no specific symptoms of prostate cancer. Certain urinary symptoms may suggest that you are suffering from prostate cancer. Such symptoms are: 

  • Frequent urination
  • Difficulty with starting urine flow
  • Urination that burns
  • Blood in the urine
  • Weak flow, or 'dribbling'

These symptoms always do not mean that you are suffering from prostate cancer, as these symptoms may be there when your prostate gland gets enlarged. 

Another symptom associated with prostate cancer is pain in different parts of the body. When the cancerous cells spread they cause pain around the prostate gland. There can be pain in the hips, pelvis, and lower back or upper thighs. 

Prostate gland also plays a major role in the male reproductive system. Thus, if there is any sexual dysfunction, there are chances that it may be due to prostate cancer. If you are having a problem in getting an erection or maintaining it also becomes difficult, one problem can be prostate cancer.  Sometimes you may experience painful ejaculation, which also points towards chances of having prostate cancer. 

Thus, if you experience any of the above mentioned symptoms, its right time to get to your physician. Remember that as there are no early symptoms, the moment you find any of these symptoms, you should pay a consult an urologist.
