Glaucoma is a group of diseases that causes damage to your eye's optic nerve and gets worse over time. The optic nerve is a bundle of millions of nerve fibres which carry visual impulse from the retina at the back of eye to the brain. The increased Eye pressure, called intraocular pressure, can damage the optic nerve and fasten the natural nerve fibre loss. Since nerve cells can't regenerate this damage is permanent and irreversible.

Without treatment, glaucoma can cause total permanent blindness within a few years. Because most people with glaucoma have no early symptoms or pain from this increased pressure, it is important to see your eye doctor regularly so that glaucoma can be diagnosed early and treated before significant functional visual loss occurs.

If you are over age 40 and have a family history of glaucoma, you should have a complete eye exam with an eye doctor every one to two years. If you have any risk factors, you may need to visit your eye doctor more frequently.

What Are the Symptoms of Glaucoma?

For most people, there are usually few or no symptoms of glaucoma. The first sign of glaucoma is often the loss of peripheral or side vision, which can go unnoticed until late in the disease. This is why glaucoma is often called the "Sneak Thief of Vision" or "Silent Thief of Eye Sight".

If you have any of the following symptoms, seek immediate medical care:

Seeing halos around lights
Vision loss
Redness in the eye
Eye that looks hazy (particularly in infants)
Nausea or vomiting
Pain in the eye

There are several risk factors, which may lead to glaucoma, such as

Age of the individual
Ethnicity- African, Caribbean, Hispanics and Asians are at higher risk
Increased eye pressure or IOP History of glaucoma in family or relatives
Presence of Myopia or Hyperopia
Previous eye injuries
Use of steroids- as injections, tablets, eye or ear drops or ointments or protein supplements for muscle building in gyms or inhalers
Previous history of anemia
Individuals conditions affecting blood flow like hypertension, diabetes or high blood sugar, migraine etc are also at risk.
Treatment-

The treatment for glaucoma relies on the nature and seriousness of every case. Generally, glaucoma cannot be totally cured, but it can be controlled. Eye drops, pills, laser strategies, and means of surgery are utilized to maintain and prevent further harm from happening. What actually is Retinitis Pigmentosa? 
Retinitis Pigmentosa (RP) refers to a group of diseases, which causes a slow, but progressive vision loss. It is a genetic disorder that affects the ability to respond to light.This inherited disease causes a slow loss of vision, beginning with decreased night vision and loss of peripheral (side) vision and eventually results in blindness.

Causes:
Retinitis pigmentosa is often hereditary (runs in families). If you or your partner has retinitis pigmentosa, there may be up to a 50 percent chance that you will pass it on to your children. Ask your ophthalmologist about genetic counseling if you are planning to have children.

Symptoms:
1. Slow loss of vision
2. Beginning of decreased night vision
3. Loss of peripheral vision
4. Difficulty in identifying colours

Diagnosis:
If you have poor night vision or a loss of side vision or if there is a history of retinitis pigmentosa in your family, your ophthalmologist will conduct a comprehensive eye exam to determine if you have retinitis pigmentosa. Your ophthalmologist will dilate your eyes to look at the back of them for signs of disease.

Treatment:
Currently there is no known cure for retinitis pigmentosa. However, research has shown that vitamin A palmitate may slow the progression of certain forms of RP. Your ophthalmologist can advise you about the risks and benefits of vitamin A palmitate and how much you can safely take. Taking too much vitamin A palmitate can be toxic, and evidence of vitamin A palmitate effect on RP progression is not substantial.
Another recommendation for slowing vision loss from RP is to wear sunglasses to protect your retina from harmful ultraviolet (UV) light.

Retinal prosthesis is also an important area of exploration because the prosthesis, a man-made device intended to replace a damaged body part, can be designed to take over the function of the lost photoreceptors by electrically stimulating the remaining healthy cells of the retina.

For reading, both eyes have to converge and if there is weakness of convergence, extra strain has to be put on the eyes, which cause headache.

Q. What is the ideal method of refraction in young patients, I,e below 40 years?

Ideal method is to dilate the pupil with Tropicasyl plus drops. This has the advantage that the doctor can see the retina and Optic Disc. This is important to exclude any involvement of brain as the cause of headache. It also relaxes accommodation and hence provides correct refraction. In cases of Hypermetropia, it is extremely essential to dilate the pupil.

Q. What is the role of Autorefractmeter (a computer) in refraction?

It is a very important instrument to provide correct refraction, but it cannot be totally relied upon. Some opticians rely on it and give wrong numbers.

Q. What is acceptance Test?

You have refracted the eyes with Autorefractmeter or Streak Retinoscope but exact power is what a patient accepts. This is very important.

Q. What is astigmatism?

There is a difference in refraction in different meridians and it is important that a cylinder number is prescribed in correct axis. This is a difficult task and needs lot of experience.

Q. Is it important to correct astigmatism?

To relieve headache and strain on the eyes, it is very important that exact cylinder is prescribed.

Q. What is the role of exact centring of Optical Lenses?

You may give correct number, but if the centre of the Optical lenses and cornea do not coincide, satisfactory results are not obtained. To dispense correct lenses is a big art and the Optician must be qualified and well experienced.

Q. Does the quality of lens affect the visual comfort?

Yes, it is important that quality of lenses should be good, preferable fibre lenses which are Ant reflect coated. Crizal coating with capability to block the UV rays are very popular.

Conclusion: Refractive errors are important cause of headache and needs proper examination of the eyes, preferably after dilating the pupil. Astigmatic error even if too small may be the cause of headache. Spectacle dispensing is an art and a patient must select proper optician. Spectacles if advised for constant use must be used constantly to get full relief.

Family history: Inheritance surely plays a role in case of Glaucoma too. If Glaucoma runs through families of an individual, there are higher chances of getting this disease. Although there is no hard and fast rule about it, the risks of inheriting this disease are on the higher side.
Eye injury: If there is any history of previous eye injuries, the probability of the onset of Glaucoma cannot be turned down. Specially, if the injury is related to optic nerve, there are very high risks involved.
Bilateral Oophorectomy: Dual Ovary removal before the age of 40 results in deficiency of estrogen resulting in Glaucoma. Although most surgeons warn their patients of the possible risk of these procedures, many surgeries such as these have resulted in patients losing eyesight in the form of Glaucoma.
Patient with Diabetes: In case of acute diabetes, retina gets damaged and the eye starts producing new blood vessels on the iris resulting into an increased eye pressure. This condition often leads to Glaucoma due to blockage of fluid flow in the eye.
Eye Conditions: Myopia, multiple surgeries to the eye, blunt trauma etc. can lead to Glaucoma. A patient who has undergone any kind of eye surgeries suffers from inflammation. However, major inflammation is often found twisting blood flow in the eye resulting into Glaucoma.
If a patient is 40 or above: After a person reaches 40 years of age, a lot of diseases start cropping in to the body. Studies show that Glaucoma has a higher frequency in people who is more than the age of 40 or above.
High eye pressure: A patient with an eye pressure of above 21 mm HG is also at a very high risk of getting glaucoma. For a normal person the IOP lies between 10 to 21 mm HG.
Blood flow related disease: Diseases such as acute migraine, hypertension and low blood pressure which involves blood flow related problem often aggravate to glaucoma.
Ethnic Background: Statistics show that people from Afro American, Hispanics, Asian and Caribbean background have greater chances of being affected by Glaucoma.

Myth #3: if you wear glasses or contacts for a continuous period, your eyes will weaken

Fact: your eyes will not grow weaker by using corrective lenses. The prescription may change over time due to aging or absence of disease but it has nothing to do with your current prescription. Some children have correctable eye problems that do require glasses to improve the condition.

Myth #4: when you get something in your eye, rub it out

Fact: this is not true for everything that gets in your eyes. If any particle falls in your eyes dust it off. But if it is sand and small debris that gets in your eye, don't touch it. Use an eye wash for flushing it out. If an object gets stuck in your eye, don't remove it or rub your eye constantly. Visit a doctor at the earliest because some serious injuries may seem minor at first.

Myth #5: dark sunglasses can protect your eyes from the sun

Fact: the color of sunglass does not ensure eye protection. Look for sunglasses that block both uva and UVB rays. Exposure to the UV-rays of sunlight can have cumulative effects on your eyes. It increases your risk for cataracts, solar retinitis, and age-related vision loss. Even if your contacts have UV protection, wear sunglasses that block 100% of uva and UVB rays for full protection.
