1. Careful with wash: if you are using a cloth diaper, be extra careful in washing such that there is no soap remaining in the cloth.

2. Clean well: when bathing your baby, make sure you clean the area covered by diaper with a mild soap and do not rub the area while drying, since the skin is sensitive.

3. Remove soiled diaper instantly: this is the most important. However busy you are, you need to aware of the times when the diaper/cloth has been soiled. And when soiled, the diaper has to be changed at the earliest. Do not keep your baby in the diaper for all hours of the day. It is good if they are out of the diaper for some time every day.

4. Keep the diaper loose. Also, ensure that you wash the cotton diapers with fabric softeners and try to avoid wipes altogether as they have strong chemicals in them.

5. Keep a tab on rashes: if your baby does get the rashes, check with the pediatrician at the earliest. Frequent topical applications of a bland protective barrier agent (zinc oxide paste) may suffice to prevent dermatitis. There are also chances of fungal infections. 5 Simple Ways To Avoid Diaper Rash For Your Baby

The primary or milk teeth begins to erupt at about 6 months of life with the complete set in place by 2.5 years of age, and the entire set is replaced by the age of 14. The benefits of some of the preventive dental measures are outlined below.

Maternal care during pregnancy: the teeth begin to form during the second trimester of pregnancy. It is very important that the maternal diet includes sufficient amounts of calcium, potassium, and fluoride for optimal tooth mineralization. Good tooth structure has greater resistance to decay than hypomineralized teeth.

Routine oral hygiene: for the first 6 months, though there are no teeth, after each nursing, end it with a spoonful or two of water to wash down the milk. Gradually, the gums can be just wiped with a gauze pad or soft cotton to remove any residual bacteria. Once the teeth begin to erupt, the nursing habits also need to be modified to ensure the baby is not allowed to go to sleep with a bottle. This is a common practice to put the baby to sleep and should be avoided to reduce the occurrence of nursing bottle caries.

Fluoride application: fluoride has been shown to have significant benefits in preventing caries. Once routine dental visits begin, then the dentist will be able to tell if fluoride needs to be applied. This can happen either in the form of a gel or varnish that is applied in the dental office or as a paste that is used at home. This helps in building resistance to decay.

Pit and fissure sealants: the tooth has a number of pits and fissures which are 8 times more vulnerable to decay than other surfaces. Deep pits are shown to harbor more bacteria and thereby greater incidence of caries. There are sealants which are thin resins that can be applied on the tooth, which can reduce bacteria accumulation and further decay.

Space maintainers: in the event that a child has lost a tooth, either due to trauma or decay and subsequent infection, then a space maintainer should be placed within the next 3 months. This helps in maintaining the space and establishing a good bite during the transition phase and later, once the permanent teeth are in place.

Orthodontic treatment: if there is malocclusion, then early intervention helps by reducing treatment time and getting better results as the teeth and periodontium are still very elastic and are more malleable to movement.

By doing these preventive measures early in life, the result is a child who has healthier teeth, less decay, less pain, and a happy smile all the time!

The appendix is a small, finger-shaped pouch attached to the large intestine in the right belly area. It is a vestigial organ as it has no specific role to play in humans, but the organ is still seen, though in a very small size compared to the earlier living beings in the evolutionary chain. Acutely inflamed appendix is the most common cause leading to it removal, often seen in the ages of 10 to 19.

Causes: the appendix gets infected by two main reasons - general infection in the abdomen that reaches the appendix or blockage of the appendix leading to inflammation and swelling within it. The appendix is a blind pouch, and there is a good chance for its blockage from food particles, lymphatic tissue, or even sometimes feces. Some of the potential risk factors for appendicitis include a diet low in fiber, high in sugar, gut flora, and family history.

Symptoms/diagnosis: in adults, the appendicitis has very characteristic symptoms including acute pain in the right upper part of the belly associated with fever and vomiting. However, in children, the pain may not be as tell-tale a sign but is still quite diagnostic of appendicitis. However, presence of the following symptoms together is surely indicative of appendicitis.
1. Right abdominal pain, especially rebound tenderness, where pressure placed in the right upper part of the belly and released leads to excruciating pain.
2. Fever, nausea, and vomiting
3. Abdominal fullness or bloating
4. Elevated white blood count (as with most infections)
Additionally, the younger the child, the symptoms are not very clear, but ultrasound will confirm the diagnosis. The inflamed, enlarged appendix will be visible on the images and could be surrounded by free fluid. Ct scan also can be considered if required to confirm the diagnosis.

Treatment: as noted above, children present with symptoms that do not pinpoint to appendicitis. Treatment usually takes two routes:

If diagnosed as appendicitis before rupture, then surgical removal is the best method to contain its symptoms. Other symptoms like fever and nausea and vomiting usually subside a couple of days after the surgery.
If the appendicitis goes unnoticed and ruptures, then the intestinal cavity can get infected, which is called peritonitis. Earlier, the preferred approach was to control the infection and then go for removal. However, lately, removing the appendix followed by antibiotics to control the infection is the preferred approach.
The prognosis and recovery from appendicitis is very good. Deaths have occurred only in very small infants, where they are not able to pinpoint the area and therefore it can go undiagnosed, leading to rupture and subsequent death.

Early identification is the key to proper identification, immediate treatment, and complete recovery from appendicitis.

Appendicitis in Children

A child s tantrums, especially during teens, are quite common. However, there could be some children who could be exhibiting an extreme version of these symptoms. This is known as oppositional defiant disorder.
Children with this disorder become easily irritable, angry, argumentative, defiant and feel vindictive against most elders (parents, teachers, and others). While this is something very common and can be ignored to be a part of teenage tantrums, the issue is when these symptoms do not seem to end. If they persist for beyond 6 months, it is time to worry. These may then begin to interfere with their daily activities including schooling, where they may not be easy for the teacher to manage.

Diagnosis of ODD: With the changing behaviour of teenagers, it is often difficult to pinpoint and say there is ODD. However, some guidelines for diagnosis are listed below. Angry/irritable, argumentative, defiant and vindictive. If these symptoms are seen for more than 6 months with no inducing reason, happens with non-siblings, and is affecting learning and playing, it is highly likely the child has ODD.

These symptoms can occur at home, at school, or in other settings   seen respectively in one, two, or more settings. Some of the symptoms are listed below.

Repeated temper tantrums
Anger bursts, swearing, using obscene language
Extremely argumentative, especially with people in authority (teachers, parents, etc.)
Annoying others and getting annoyed easily
Noncompliance to rules and regulations at school and institutions
Defending one s mistakes and blaming others for it
These result in poor academic performance, antisocial behavior, substance abuse, and higher suicidal tendencies.
Treatment

Treatment depends on the presenting symptoms, the age of the child, and supportive care available. The child should be able to actively take part in psychotherapy to reap good benefits. It would otherwise be a task with no results.

Psychotherapy will help the child improve its cope and express and control anger. This also improves problem-solving skills.
Cognitive-behavioural therapy tries to mould the behaviour.
If required, the parents also would be involved to improve family s involvement in the treatment. Caretakers are given special training if required so that they can support in long-term medical care.
The child also needs to be trained for appropriate behaviour under different circumstances.
Rewards for positive behaviour and punishments for negative behaviour are useful ways.
Prevention: Early identification can help in minimizing distress to the family and help in the early arrest of the disease. The family is also taught basic and simple steps which can help in supporting therapy. Early rejection at school and loss of learning, can happen which can be managed with early intervention. A nurturing and supportive family can help manage the child very well.
