Knowing Premature Ejaculation - PART 2: Diagnosis, Types & Causes

As many as 30-40 % men across the world including India experience problem of PE at some time of life. In Part 1, we learnt about the condition called Premature Ejaculation (PE). In this 2nd part, let’s understand about it’s diagnosis & types of PE - why and how it happens.

How to Diagnose PE:

The specific criteria for premature (early) ejaculation are as follows:
- In almost 75-100% sexual activity, the experience of ejaculation occurring during sexual intercourse within 1 minute after vaginal penetration and before the individual wishes it.
- The problem has persisted for at least 6 months and is a cause of mental stress to the person.
- The dysfunction cannot be better explained by any other nonsexual mental disorder, any medical disease, the effects of a drug or medication, etc
 
Severity of PE:
 
The severity of premature (early) ejaculation is broadly defined as follows:
1. Mild (occurring within approximately 30 seconds to 1 minute of vaginal penetration)
2. Moderate (occurring within approximately 15-30 seconds of vaginal penetration)
3. Severe (occurring even before sexual activity, at the start of sexual activity, or within approximately 15 seconds of vaginal penetration). In such cases conception will not be possible unless artificial insemination is used.
 
TYPES and Characteristics of P.E. :
Premature ejaculation can be Chronic (lifelong) or Acquired (recent).
Chronic PE:
With chronic (lifelong) premature ejaculation, the person has been experiencing premature ejaculation since he became sexually active (ie, post puberty). 
Acquired PE:
Acquired (recent) premature ejaculation means that the condition began in an individual who previously experienced an acceptable level of ejaculatory control and only recently has developed PE.

What are the Causes?
The causes of PE can be divided into two broad sub-heads, which are psychological or biological cause:
 
 
1. Psychological Causes:
Premature ejaculation is believed to be a psychological problem and does not represent any known organic / physical disease involving the male reproductive organs or any known defect in the brain or nervous system. It is usually due to a pattern that is hard to change and is a result of your previous sexual experiences.
 
One of the most common reason is childhood habit of reaching climax/ ejaculation quickly because of fear of discovery when masturbating or during early sexual experiences with a female partner. This pattern of rapid attainment of sexual release is difficult to change in later stage of life (in marriage or long-term relationships).
 
Other reasons are situations in which one may have hurried climax/ ejaculation in order to hide any problem or feelings of guilt that make you rush through sexual encounters. Also, psychological causes include anxiety and relationship issues which can also result from deep anxiety about sex that relates to bad experiences encountered by the patient during development (eg: incest, sexual assault, conflict with parents, etc ).
 
2. Biological Causes
 
A number of researchers have found differences in nervous stimulations and hormonal differences in men who experience premature ejaculation compared with individuals who do not.
 
Some believe that some men have hyper-excitability or oversensitivity of their genitalia.
 
Abnormal functioning of the ejaculatory system can be attributed to:
- Thyroid problems; infection or inflammation of the urethra or prostate.
- Nerve damage occurring due to trauma or surgery (a very rare cause).
- Abnormal levels of hormones and/or neurotransmitters (chemicals present in the brain)
- Higher free and total testosterone levels or varied other biochemical markers
*The research however has been non-conclusive.

Knowing Premature Ejaculation - PART 2: Diagnosis, Types & Causes

Most families refer to their newborn baby as their 'bundle of joy'. The news of a child being born brings immense joy to the entire family. However, due to various reasons, a child could be born with some medical abnormalities, which would be known as congenital abnormalities. There are several different types of heart defects that can be congenital. These usually manifest themselves either immediately after birth or in the early years of life. In some cases, the abnormality could be detected on prenatal ultrasounds. In others, it may not be and the family could be caught off guard about the condition. This causes a lot of stress, both for the child, who does not receive regular postnatal care and for the parents immediately after the delivery process.

If the baby has the following symptoms within the first few hours of life, there could be a serious underlying condition, which requires medical attention. The presence and severity of the symptoms would depend on the actual abnormality.

The skin is pale gray or blue in color due to excessive venous flow in the system
Excessive sweating
The child is exerting to breathe regularly
Rapid breathing causes added load on the heart accompanied by a grunting noise
Flared nostrils i.e. the baby attempts to take in more oxygen with each breath causes flared nostrils
Swollen legs, eyes, and abdomen: Fluid retention in the legs and abdomen is quite common, and this could be characteristic of newborns with congenital heart disease
Shortness of breath, even during feeding
Clubbed fingernails
Lethargy and low energy, even with feeding, therefore very poor feeding pattern
Chest pain, which may cause the newborn to cry incessantly
Low weight gain, as they feed less
In some children, symptoms manifest only during the teenage years or early adulthood. These conditions are not very severe and the symptoms include:

Swelling of the hands, feet, and ankles due to fluid accumulation
Lowered energy levels, leading to easy fatigue
Shortness of breath with even minimal physical activity
Inability to exercise
Developmental delays and changes in growth milestones
Recurrent respiratory tract infections including sinus infections, bronchitis, and pneumonia
Endocarditis
Pulmonary hypertension
Heart failure, where the heart is not able to effectively function and pump blood to all parts of the body.
Some or more of these symptoms should trigger a warning to get the child tested for congenital heart disease. While some would just require a monitoring until severe symptoms develop, severe conditions like holes, abnormal valves, narrowed arteries, and blood vessel abnormalities might require immediate intervention.

Women have a higher risk of suffering from this infection as compared to men. This is because women have a shorter urethra than men and hence the bacteria needs to travel a shorter distance to reach the bladder. Urinary tract infections can usually be treated easily with a course of antibiotics as long as they are detected in time. If not detected in time, it can affect the kidneys which make it a more serious problem. Hence, it becomes important to be able to recognise the signs of this infection. 

Some of the symptoms to watch out for are:

A persistent urge to urinate but low volume of urine produced
A burning sensation while urinating
Cloudy urine
Blood in the urine which makes the urine appear red, pink or blackish
Strong smell of the urine

A urinary tract infection that affects the urethra is also known as urethritis. A burning sensation when urinating and discharge along with urine are the most prominent symptoms of this type of infection. When this infection affects the bladder it comes to be known as cystitis. Cystitis patients typically suffer from frequent and painful urination along with discomfort in the lower abdomen and pressure in the pelvic area. They may also notice blood in the urine.

Acute pyelonephritis refers to a urinary infection that affects the kidneys. In such cases, along with the typical symptoms, the patient may also experience high fever, nausea, shivering and pain in the upper back and sides of the abdomen.

This infection can affect a person at any age. Most women suffer from at least one bout of urinary tract infections in their lifetime. Some of the factors that increase the risk of suffering from this infection are:
Certain types of birth control such as diaphragms and spermicidal agents

Menopause
New sexual partners
Urinary tract abnormalities
Kidney stones or other blockages in the urinary tract
Weakened immune system
Use of catheters
Recent urinary procedures that involve the use of medical instruments
Urinary tract infection can be easily prevented by living a healthy lifestyle. Drink plenty of fluids to flush bacteria and other toxins from your body. Bacteria multiplies in the bladder if urine stays for too long. Hence, urinate frequently and do not try to control your bladder. Avoid synthetic underwear and wear cotton underwear with loose fitting clothes that keep the area around the urethra dry.

How to prevent recurrent urinary tract infections
A urinary tract infection or UTI can be quite painful and uncomfortable. This infection can be described as an infection that affects the ureter, urethra, bladder or kidneys. Women have a shorter urethra than men which makes this more vulnerable to this kind of infection. It is said that most women suffer from at least one bout of this infection in a lifetime. Urinary tract infections are caused by bacteria and may often recur after treatment. However, in most cases, this can easily be prevented. Here are a few tips to help prevent recurrent urinary tract infections.

Drink plenty of fluids: Ideally, we should drink 8-10 glasses of water a day at regular intervals. This keeps the body hydrated and flushes toxins and bacteria from the body. Water also prevents the formation of kidney stones that could increase your risk of suffering from a urinary tract infection.
Urinate frequently: If you drink the required amount of water, you will typically need to urinate frequently. Do not try to control your bladder. Holding your urine in your bladder allows bacteria to multiply inside the bladder. This is an easy way for a UTI to begin.
Follow healthy bathroom habits: Cleanliness is the first step to preventing any infection. When using public restrooms, flush the toilet before and after urinating. In the case of women, it becomes important to ensure that the seat of the toilet is clean as well. After urinating wipe the residual urine with toilet paper in a motion that goes from front to back. Since the urethra is situated very close to the rectum, this step is very important. For uncircumcised men, it is important to wash the foreskin after urination.
Wear loose clothes: Tight fitting clothes trap moisture and aid in the multiplication of bacteria. Hence, always wear loose fitting clothes that keep the area around the urethra dry. Also, avoid synthetic underwear and only wear cotton underwear.
Urinate after intercourse: The relative closeness of the vagina to the urethra makes it easy for bacteria to enter the urethra during intercourse. Hence, make it a habit of washing your pubic region after urinating. This helps flush out any possible bacteria that may have entered your body.
Choose the correct method of birth control: Certain types of birth control such as diaphragms and spermicidal agents can promote a urinary tract infection. Hence, it is best to avoid these types of birth control and pick alternatives.

Why it's more important to brush your teeth in the night than in the morning

Most people wake up every morning and stumble to wash their teeth first thing. Do you believe that brushing your teeth in the morning is sufficient to keep your mouth healthy?

After you have your dinner just rinsing your mouth is not enough. Tiny food particles that get stuck in your teeth rot overnight. It is best to go to bed with a clean mouth rather than a mouth with tiny decaying food particles.

Is it More Important to Brush Your Teeth in the Night Than in the Morning?

Most women get accustomed to and know the pattern of their menstrual cramps. So, when something out of this pattern happens, they know it is not normal. There are various reasons for pelvic pains not associated with menses. The following are some common causes and it is always advisable to seek medical help, which will help in right diagnosis, early intervention and reduced complications.

1. Ectopic pregnancy: If a fertilized egg has planted itself outside of the uterus or the womb, it is referred to as ectopic pregnancy. The fallopian tube is the most common place for ectopic pregnancies. There is associated pelvic pain, which is unilateral, sharp, and gradually worsens. There would also be associated bleeding which is darker than the regular bleeding. This again should be confirmed on an ultrasound and then the ectopic pregnancy removed.

2. Endometriosis: The inner thick layer of the uterus could be infected leading to severe pelvic pain, especially in women who are in their 30s, who are not able to conceive. This is often ignored, considering it as normal and it continues to progress and could even result in infertility.

3. Pelvic inflammatory disease (PID): Infections like gonorrhea or chlamydia travel up from the genital tract and reach the uterus. Infection along the entire reproductive tract can lead to severe pain. In most cases, if identified early, a course of antibiotics is all that is required to control this infection. Left untreated, it can even cause infertility.

4. Ovarian cysts: Another common cause of pelvic pain, they are quite often asymptomatic, except for occasional pelvic pain. They also disappear with no medical intervention. Sometimes, however, it may get infected or rupture and can cause severe pain. This can is an indication for medical intervention. Ultrasound may be required for diagnosis and treatment may include surgical removal.

5. Uterine fibroids: The fibroids are noncancerous growths, which can grow to a considerable size and fall short of blood supply. This may cause them to burst, which can be very painful.

6. Miscarriage: Most miscarriages occur before 13 weeks of pregnancy and the usual accompanying symptoms of bleeding are severe pelvic cramps. If you are pregnant and notice bleeding with pelvic pain, it is advisable to immediately visit a doctor. Diagnosis may require an ultrasound to confirm the miscarriage. If medications are not sufficient to remove the remnants, then a dilation and curettage may be required.

7. Non-gynecological causes: Appendicitis, urinary tract infections, hernia, lower spinal problems, irritable bowel syndrome can also cause pelvic pain. The cause should be identified and managed accordingly.

Unfortunately, most women continue to experience and suffer with chronic pelvic pain without a cause identified. A thorough internal examination can help identify the problem and treat the issue.
