Handling an Auto Immune Disease

What are Autoimmune Disorders?
It is a disorder of the immune system where the immune system can get ‘hyperactive’ or on the other end of the spectrum can become exceptional slow in its response to germs and bacterium, leaving the individual vulnerable to a host of infections and diseases.
When in ‘Hyper mode’ the body’s immune system starts producing antibodies (White Blood Cells) attacking and damaging the body’s own tissues (Red Blood Cells) beyond repair. Treatment of the Auto-Immune diseases generally focuses on reducing the activity levels of the immune system so as to let the Red Blood Cells multiply and heal the body to an extent.
Not all Auto Immune diseases are life threatening or terminal in nature. Some are even classified as “commonly occurring” disorders in the larger population. Examples of these would be –
1) Psoriasis – A Skin disorder where overactive immune system blood cells called T-cells collect in the skin. The immune system activity stimulates skin cells to replicate rapidly, producing layers of silvery scaly plaques on the skin. These often look like ‘Dandruff’ and can be seen all over the individual’s body, accompanied by severe itching and skin irritability. Psoriasis is often triggered due to stress and hormonal imbalances along with other environmental factors such as poor hygiene, inadequate diet, lack of sleep etc. Along with oral medication, lifestyle changes such as reducing stress at work or home, making sure one gets at least 8 hours of restful sleep per night and regular head and body massages are known to help reduce the severity of the problem to a great extent.

2) Rheumatoid Arthritis – The hyper immune system produces antibodies that attach themselves to the joint linings and then start to eat away at the cartilage and tissues causing swelling or inflammation and a sharp stabbing continuous pain. Oral medication along with gentle cardio (slow walking) and stretching exercise routines such as Yoga, Tai Chi and Pilates are known to have countered the ill-effects of this movement restricting disorder, to a large extent.


3) Aggravated Thyroid – An aggravated thyroid is a result of the excessive antibodies produced by an overactive immune system which attack the thyroid gland and slowly destroy the cells that produce the all-important thyroid hormone. Prolonged negligence or when left undetected for long, it often leads to Grave’s Disease (linked to hyperthyroidism) or Hashimoto’s Thyroiditis (linked to hypothyroidism.) The former can lead to weight-loss, bulging eyes, rapid heart-rate, and chronic fatigue while the later often causes sudden weight gain, depressed state of mind, dry skin and severe constipation.  A good way to avoid diseases of the thyroid would be to maintain a healthy active lifestyle and a stable healthy body weight. It is also recommended that one goes in for regular preventive health check-ups for early detection of any thyroid related disorders.

A little can go a long way in limiting ill effects of autoimmune disorders.

 Movement disorders    the term refers to a class of neurological conditions, which lead to either an increased/abnormal movement or a scarcity of involuntary and voluntary movements.

Types of Movement Disorders- 
The most common types of movement disorders are  
  Myalgia   More commonly known as muscle pain, myalgia originates in any part of the body. A person with myalgia will experience achiness and soreness in the muscles, which may range from severe to mild.

Myalgia does not occur from an underlying disease. Rather, the pain develops over time, mostly due to prolonged sitting, lying down, a strenuous physical activity, strains or sprains.

  Muscular dystrophy   This refers to a group of genetic disorders that lead to weakness and loss of muscle mass. The abnormal genes cause muscle degeneration and usually begins in early childhood. Myotonic, Facioscapulohumeral (FSHD), and Limb-girdle are a few forms of muscular dystrophy.

Muscular dystrophy is characterized by symptoms like   difficulty jumping and running, muscle stiffness and pain, walking on toes, waddling gait, trouble swallowing and breathing etc.

  Dystonia   The condition involves continuous involuntary muscle contractions with repetitive, twisting movements. Dystonia may affect either a specific body part (known as focal dystonia) or the full body (known as generalized dystonia).

  Huntington s Disease   This is an inherited, neurodegenerative condition, where the brain s nerve cells break down over time. Huntington s disease usually kicks in, in the 30s or 40s. The disease results in psychiatric and cognitive symptoms   amnesia, confusion, slowness in activity etc.

  Multiple System Atrophy   This is a rare neurological disorder, which affects both the autonomic nervous system   the part of the nervous system that controls involuntary actions like digestion and blood pressure   and movement. Multiple System Atrophy leads to movement disorders like Parkinsonism or ataxia.

Symptoms of this disorder include   double or blurred vision, visual disturbances, unsteady gait, loss of balance etc.

  Tourette Syndrome   This is a disorder of the nervous system, which occurs usually between one s childhood and teenage years. The disorder is associated with vocal sounds (vocal tics) and repetitive movements (motor tics).

A headache can be a hindrance to any person at any time. It can either occur on left or right side in case of a migraine, be isolated to a point or radiate across the entire head. The pain can be sharp, throbbing or dull. It can either develop gradually or suddenly and can last for few hours to several days. Some of the possible causes of secondary headaches include arterial tears, acute sinusitis, brain aneurysm, brain tumour, dehydration, ear infection, carbon monoxide poisoning, dental problems, glaucoma, hangovers, flu, hypertension, stroke, panic attacks, toxoplasmosis, post-concussion syndrome, trigeminal neuralgia, monosodium glutamate, meningitis, giant cell arteritis etc. Some of the common triggers of secondary headaches include ice cream headaches, compression headaches, thunderclap headaches, sinus headaches, rebound headaches, spinal headaches etc.
When to seek emergency care for headaches?
Headaches could indicate serious disorders such as encephalitis, stroke or meningitis. It is advisable to seek immediate medical care for an unbearable headache which also includes the inability to understand speech, stiff neck, trouble in vision, vomiting accompanied with nausea, trouble in speaking, high body temperature, greater than 104 F. A doctor should be consulted if the pain occurs multiple times in a short duration, keeps one from working or performing daily activities, the pain doesn t improve even after the consumption of over the counter medicine and disturbs sleeping.

Psoriasis is the condition in which the infected skin cells multiply 10 times faster than the normal rate. During this disease, the normal skin tends to get covered by a layer of dead skin cells, not allowing the skin to breathe. Kidney disease: According to a 2013 study, published in the British Medical journal people with psoriasis have an increased risk of encountering kidney disorders. Severe and mild psoriasis increases the likelihood of kidney diseases by 58% and 36% respectively.
Obesity: People suffering from mild psoriasis are 30% likelier to be obese than those who are not. For people diagnosed with severe psoriasis the chances increase by 80%.
Diabetes and high cholesterol: High levels of triglycerides, total cholesterol and lipoprotein are present in patients with psoriasis, and these patients are 28% likelier to have high cholesterol compared to the general masses. According to a recent study, people with the condition have a 46% increased chance of suffering from Type 2 diabetes.
Peptic ulcers and mood disorders: In some cases, peptic ulcers are also linked with psoriasis. The reason for this is attributed to inflammation. Psoriasis also leads to mood disorders like depression due to the pain and anxiety arising from the condition.

Myofascial pain syndrome refers to a chronic condition that affects a person s musculoskeletal system. With this disorder, pressure on the sensitive points (known as trigger points) in the muscle induces pain in various parts of the body.

Myofascial pain syndrome mostly affects people in the age group 40-60 years.

What causes myofascial pain?

Typically, myofascial pain syndrome occurs because of repetitive muscle contraction or overuse, or due to an injury. Sensitive areas of the tight muscle fibres can develop following an injury/trauma, and ultimately lead to pain and strain throughout the muscle.

Symptoms to watch out for-

Common signs and symptoms of myofascial pain syndrome include-

  Severe pain in localized muscle areas
  Pain that worsens when you stretch or strain the affected muscle
  Muscle pain that persists for longer than usual, or worsens with time
  Painful, tender knots in the muscles that lead to severe pain when pressed
  Weakness and stiffness in the muscles
  Inflexibility or a reduced range of motion

Although muscle pain is temporary in most cases, and usually subside on its own, you should visit a doctor immediately if the pain becomes intense and severe. Otherwise, the symptoms of myofascial pain syndrome can aggravate and impede your daily routine.

What is the treatment for myofascial pain syndrome?

Treatment for myofascial pain syndrome include a range of pain management options such as the following-

  Medications
Your doctor will likely prescribe medications that include anticonvulsants, analgesics, and over-the-counter Non-steroidal Anti-inflammatory Drugs (NSAIDs). These work as painkillers and help relieve pain and reduce muscle spasms.

  Dry-needling
Dry-needling is a quicker way of getting rid of myofascial pain symptoms. The doctor inactivates the trigger points by directly inserting a needle into it. He/she will then slowly move the needle around and poke it inside and out. The initial process can be a little painful, but it certainly helps manage the pain in your muscles. Alternatively, acupuncture needles can be used for the same procedure since they are small and are less painful.

  Local injections
Local injections, also known as trigger point injections, are similar to dry-needling. The only difference is that local injections involve injecting a solution (saline/steroid/or a local anaesthetic) into the trigger points. The method yield similar results as does dry-needling.

  Therapy
Different types of therapies can be used to ease the pain associated with this chronic pain disorder.

Ultrasound therapy where sound waves are transmitted into the muscle tissues through a sound-conducting gel applied on the skin. The sound waves heat up and relax the muscles, thereby improving blood circulation and removing the scar tissues. Ultrasound therapy is also effective in reducing muscle stiffness and increasing one s mobility.
Massage therapy also is very effective in treating myofascial pain syndrome. The therapist usually uses his/her fingers or some sort of equipment to induce pressure on the various trigger points. When performed, this procedure may first aggravate the muscle pain and then release muscle tension.
Apart from these, the doctor may prescribe regular exercising or physiotherapy treatment for patients suffering from chronic muscle pain.
