When dieting and exercising fail to help an obese person in managing his/her weight, bariatric surgery may be suggested. One of the most popular types of bariatric surgery is a gastric sleeve surgery or sleeve gastrectomy. The main aim of this surgery is to reduce the amount of food that can be consumed in one sitting. It does not affect the amount of nutrients and fats absorbed from the food that has been eaten. A majority of the weight loss resulting from this surgery happens within the first 6 months following which there is a slower weight loss over the next 6 months.

A gastric sleeve surgery is a relatively safe and short duration surgery. Prior to the surgery, you would have been given a special diet to be followed, the duration varies between one week to two weeks. It is essential to follow this diet strictly. This surgery may be performed laparoscopically, through tiny keyholes. More than half the outer side of the stomach is removed. The remaining part of the stomach takes the form of a tube. This surgery is not reversible. 

Recovery from a gastric sleeve surgery is quite smooth and varies from person to person. A certain amount of pain and discomfort is normal for the first and second day following the surgery. The patient is put on clear liquids for the first 2 weeks following surgery. Soft foods are introduced in week 3 and 4. You can get back to small amounts of your normal food in week 5. You will notice that you feel fuller much faster and are able to consume a very limited portion of your normal food intake. In some rare cases, your food may also be emptied into the small intestines at a very fast rate. This is known as dumping syndrome. This may cause diarrhea and nausea and leave you feeling weak. It is essential to drink plenty of water to allow for smooth bowel movements and to prevent constipation.

On an average, this surgery can help people lose between 60 to 90% of their excess body weight. However, this requires control over your diet and the amount of exercise you do. At some point in time, your weight loss may plateau. This is normal to overcome this, you must make a change to your diet or exercise schedule. A dietician can also help you address the right number of meals needed by your body. It is very important not to fall back to your earlier unhealthy lifestyle and to maintain a positive attitude.

Vascular disorders are disorders of the vessels in your body that carry either blood or waste products. Peripheral Venous Disease: Peripheral venous disease is characterized by damage to the valves that allows blood to flow in a single direction. Damaged valves can cause blood to flow backwards and therefore, accumulate. Treatments for this disorder are incorporating certain lifestyle habits such as quitting smoking and alcohol.
Blood clots in the veins: Blood clots may occur in the veins present inside the muscles of the thighs and lower legs which lead to deep vein thrombosis. You may be prescribed anti-coagulants, also known as blood thinners, to treat this disorder.
Causes

In very severe cases, the patient may also complain of blood discharge while coughing, high fever, rashes on the skin, neck stiffness and other such conditions. 

Causes: there are various causes of congestion. While infection, allergy and cold are some of the most common conditions that may cause congestion, it has also been seen that congestion may arise as a symptom of a more serious ailment. Asthma is one such respiratory ailment that causes swelling in the airways, which can give rise to congestion. Also, inflammation of the bronchial tube lining can cause this condition where a greenish discharge happens due to the viral nature of the condition. Pneumonia and tuberculosis are also ailments, which can cause severe and painful congestion. 

Remedies: as per ayurveda, there are many remedies that can help in treating this condition. One can drink herbal tea all day as the anti-oxidants will help in flushing out all those toxins that are causing a painful build-up of mucus and fluids inside. Also, one can use essential oils like eucalyptus oil for inhalation, steaming and even massages. One must take care not to let this oil touch the hair as greying might happen. Yoga and aromatherapy can also be practiced so as to do away with congestion. Also, the patient can add a pinch of salt to a glass of warm water in order to do a saline water gargle which will eventually clear the air passages. 

Taking care of one's diet is one of the most important parts of any treatment in ayurveda. Ingesting warm liquids like milk and warm food can help in loosening and removing the mucus build up.

Ayurvedic Medicine for Chest Congestion Treatment

GERD is a digestive disorder that affects the lower esophageal sphincter, which is the ring of muscle between the esophagus and stomach. Gastroesophageal reflux is the backward flow of acids from the stomach into the esophagus. Doctors believe that some people usually suffer due to a condition called hiatal hernia. Pregnant women usually suffer from heartburn or acid indigestion caused by Gastroesophageal Reflux Disease.

The primary goals in the treatment of this disease are relief from symptoms, healing of erosive esophagitis and prevention of complications of esophagitis. The symptoms include heartburn and regurgitation. The setting includes a proton pump inhibitor (PPI). Patients suspected due to GERD with non-cardiac chest pain are recommended to have diagnostic evaluation before institution of therapy. For diagnosing GERD, barium radiographs should not be used.

The other points that must be remembered to diagnose GERD are
1. Endoscopy is recommended only in case of alarming symptoms. Even in typical symptoms, the upper endoscopy is not recommended.
2. For preoperative evaluation, esophageal manometry is recommended, but it has no role in diagnosing GERD.
3. Ambulatory reflux monitoring is a test that can assess association of reflux symptoms.

Management:
The suggested lifestyle modifications for the patients with GERD are

    Weight loss is recommended for patients who are overweight. So, avoid large meals and decrease dietary fat intake.
    Avoid alcohol, chocolate, onions, garlic, acidic foods and peppermint.
    Do not lie down within 3 to 4 hours after meal (recommended for nocturnal GERD patients).
    For symptom relief and to heal erosive esophagitis, a 8-week course of PPIs is the therapy of choice.
    Medications that help in potentiating GERD symptoms, including alpha-adrenergic agonists, nitrates, calcium channel blockers and theophylline should be avoided.
    For patients without diagnostic evaluation, therapy for GERD other than acid suppression, including prokinetic therapy should not be used.
    For patients with partial response to PPI therapy, additional symptom relief can be achieved by increasing the dose twice daily or switching to a different PPI.
    It is recommended to elevate the head of the bed 10 to 20 cm.
    Avoid wearing clothes which are tight around the waist.
    If patients experience heartburn relief, Histamine-receptor antagonists (H2RA) therapy can be used as a maintenance option in patients without erosive disease.

Some surgical options
1. For long-term therapy for patients suffering with GERD, surgical therapy is the best treatment option.
2. In patients who have no evidence of erosive esophagitis, preoperative ambulatory pH monitoring in mandatory.
3. Gastric bypass is the preferred operation for patients who are suffering with obesity and are contemplating surgical therapy for GERD.

Apart from these, there are potential risks that are associated with PPIs such as switching them can be considered in setting of side effects and the therapy can be a risk factor for Clostridium difficile infection.

Management of Gastroesphageal Reflux Disease (GERD)

The lungs take in oxygen and leave out carbon dioxide through breathing. It often does not get noticed but is the most vital function. When a person is not able to breathe properly, then the overall physical ability of a person is reduced. Even walking a few steps or doing simple chores can seem daunting tasks. Therefore, shortness of breath can be very limiting and may alter the quality of life of the patient.

Some techniques to reduce shortness of breath are:

Breathe by reducing the rate of respiration. This reduces shallow breathing, which often happens, and so improves the amount of oxygen that reaches the system with each breath.
The patient should learn to relax, sit, and pace around without exertion.
Become aware of air movement during respiration   controlled breathing and active cycle of breathing techniques.
Postural drainage   Depending on the area of the lung that is causing shortness of breath, the person is made to lie prone and turn to a side that will allow the fluid in that region to drain out.
Overall relaxation techniques to improve airflow and control short of breath.
Avoid bending down, as it can induce shortness of breath.
Over-eating at one go in one meal should be avoided.
Weight needs to be maintained, especially if not under control. This can lead to shortness of breath without any associated medical condition either.
The patient should learn to push objects instead of pulling them.
Heavy objects should not be lifted but moved down at ground level.
Avoid extreme hot or cold showers.
Quit smoking as it causes various lung problems.

Soak in beer laced water - Beer can be used in bubble bath too! Mix some soap with beer of your choice and add the mix to the running bath water. Soak in this water and feel your skin become soft and smooth.
Use beer as a hair-pack - Wash your hair with your usual shampoo. Now pour the mixture made of distilled water, beer, two teaspoon apple cider vinegar and few drops of essential oil (any scent), on your hair. Make sure it reaches the hair that is inside as well. Rinse this mixture with lukewarm water. You can do this once a month.
Get the beer glow - Take half a teaspoon of beer, one egg white and a drop of almond extract. Whisk them together and keep aside. On your washed and dried face, apply the mask and let it dry until your skin feels tight. Wash it off with lukewarm water.
