What are Autoimmune Disorders?
It is a disorder of the immune system where the immune system can get ‘hyperactive’ or on the other end of the spectrum can become exceptional slow in its response to germs and bacterium, leaving the individual vulnerable to a host of infections and diseases.
When in ‘Hyper mode’ the body’s immune system starts producing antibodies (White Blood Cells) attacking and damaging the body’s own tissues (Red Blood Cells) beyond repair. Treatment of the Auto-Immune diseases generally focuses on reducing the activity levels of the immune system so as to let the Red Blood Cells multiply and heal the body to an extent.
Not all Auto Immune diseases are life threatening or terminal in nature. Some are even classified as “commonly occurring” disorders in the larger population. Examples of these would be –
1) Psoriasis – A Skin disorder where overactive immune system blood cells called T-cells collect in the skin. The immune system activity stimulates skin cells to replicate rapidly, producing layers of silvery scaly plaques on the skin. These often look like ‘Dandruff’ and can be seen all over the individual’s body, accompanied by severe itching and skin irritability. Psoriasis is often triggered due to stress and hormonal imbalances along with other environmental factors such as poor hygiene, inadequate diet, lack of sleep etc. Along with oral medication, lifestyle changes such as reducing stress at work or home, making sure one gets at least 8 hours of restful sleep per night and regular head and body massages are known to help reduce the severity of the problem to a great extent.

2) Rheumatoid Arthritis – The hyper immune system produces antibodies that attach themselves to the joint linings and then start to eat away at the cartilage and tissues causing swelling or inflammation and a sharp stabbing continuous pain. Oral medication along with gentle cardio (slow walking) and stretching exercise routines such as Yoga, Tai Chi and Pilates are known to have countered the ill-effects of this movement restricting disorder, to a large extent.


3) Aggravated Thyroid – An aggravated thyroid is a result of the excessive antibodies produced by an overactive immune system which attack the thyroid gland and slowly destroy the cells that produce the all-important thyroid hormone. Prolonged negligence or when left undetected for long, it often leads to Grave’s Disease (linked to hyperthyroidism) or Hashimoto’s Thyroiditis (linked to hypothyroidism.) The former can lead to weight-loss, bulging eyes, rapid heart-rate, and chronic fatigue while the later often causes sudden weight gain, depressed state of mind, dry skin and severe constipation.  A good way to avoid diseases of the thyroid would be to maintain a healthy active lifestyle and a stable healthy body weight. It is also recommended that one goes in for regular preventive health check-ups for early detection of any thyroid related disorders.

In addition to pumping blood to various organs, the heart also has its own blood supply, through which it receives its oxygen and nutrient supply. In patients with coronary artery disease and atherosclerosis, there is a narrowing of the blood vessels which reduces the amount of blood flow to the target organs, including the heart. 

When this happens in the heart, there is chest pain due to overexertion of the heart muscles. There could be two patterns to this chest pain. With regular exertion like exercise, there would be chest pain, and most patients are familiar with this pattern.

This is known as angina pectoris or stable angina. In some people or in some instances, chest pain occurs which is sudden and not of a predictable pattern. It could be related to extreme exertion or stress. This is known as unstable angina and can lead to heart attack and be life-threatening.

Stable angina or angina pectoris has a stable, predictable pattern which most patients get familiar with over a period of time and learn to manage. Read on to know more about the signs and symptoms and management techniques.

Signs and symptoms: Stable angina usually occurs after a round of physical exertion. The patient feels a feeling of tightness in the chest which feels like the chest being squeezed. The pain can gradually spread to the shoulder, arms and even the neck. The pain can also be induced by eating, exposure to cold, emotional stress. It lasts for about 15 minutes and is relieved by rest and sublingual nitroglycerin. The pain intensity does not change with position or coughing. In addition, the patient may also experience shortness of breath, fatigue, profuse sweating, nausea, and dizziness.
The patient may be able to detect signs on further testing including ECG, echocardiography and stress testing. Features like cardiomegaly, altered ejection fraction would be detected based on the severity of the disease.

Treatment: Immediate treatment to relieve the pain includes resting and sublingual nitroglycerin. On an ongoing basis, the treatment would include 3 approaches   lifestyle changes, medications, and surgery.

Lifestyle changes: Regular exercise, smoking cessation, reduced fat intake, reduced alcohol consumption, weight loss, and stress management are some lifestyle changes to be made to improve symptoms.
Medications: A number of medications would be used depending on patient s symptoms. Aspirin to prevent clotting, medications to control blood pressure and cholesterol and diabetes.
Surgery: In patients with advanced coronary artery disease, revascularization methods may be required, which includes angioplasty and coronary bypass.
In a person with known risk factors, it is advisable to have regular checkups so that the disease progression can be controlled and symptoms managed with minimal intervention.

Predictable Chest Pain Pattern - What Does It Indicate?

Bowel movement is extremely important for the body. An abnormal bowel movement can lead to a painful, hard and dry stool. If the constipation is accompanied by symptoms such as weight loss, abdominal pain and blood in stool, it should be immediately reported to the doctor. 6. Memory and Concentration: In this phase, the patient can also experience difficulty in remembering important things and concentrating on tasks. The patient may also be preoccupied with a number of things which can prevent him or her from making proper decisions. Slow speech and lack of energy may follow as a result of such preoccupation. Preoccupation with death is a common symptom of this phase.

Most people have head or what became in the 1980s and '90s the dreaded AIDS virus. With time and the availability of medicines, it ceased to be a killer. However, the knowledge of it helped stem its spread. Most people, unfortunately, don't realize that hepatitis C is also a killer, simply because not much has been said about it. In fact, it is also a virus, but is 10 times more infectious than HIV.

Hepatitis C is transmitted through the blood, and is usually passed on to women through infected needles and sex. The virus affects 1 in every 100 people in India, while globally 180 million are infected with it. Sometimes, it may just pass through the body, like many other viruses do, but sometimes, it can remain. If detected quickly, within six months or so, cure rates are high. the problem is that it is often not easy to detect, as symptoms resemble those of a regular seasonal viral infection: fatigue, nausea, loss of appetite. If it remains in the body, becoming chronic, then it may progress to liver disease. But the hepatitis C virus (HCV) doesn't have to be a killer. You can conquer it with these moves. 

What you should do before
A number of couples who come to me have planned pregnancies-they opt for a baby, rather than the baby just 'happening' to them. Usually, in the first trimester, your gynecologist will ask you to do a simple blood test for HIV, hepatitis B and C. In the case of a planned pregnancy, visit your doctor beforehand and ask if you need to take these tests before you conceive. However, there is no vaccine for HCV yet.

What you should do after
If a woman find out in the first trimester that she is hepatitis C positive, there's nothing much that can be done, as anti-viral medications cause birth defects, so a mother can only be put on them after delivery. She is advised to continue the pregnancy. A baby's chance of acquiring the infection in utero is between 5 and 7%. While this is not high, parents may like to avoid the risk. However, co-infection with HIV (if the mother is HIV positive) pushes the risk up to 19.4%. The pregnancy itself will not be hampered by the HCV infection. Nor does the risk of transmission to the child have anything to do with the mode of delivery-either vaginal or C-section. In India, there is little data on HCV transmission from mother to child. However, once the baby is born, the pediatrician may not do an immediate test to check for the virus, as it generally clears out from the baby's system in a year or so. Testing may only be done at 18 months.
