Many of the sexual issues are primarily not big problems. Many of them are very basic issues and because of ignorance, myths, and social taboos lead to development of anxieties, distress and further worsening of the problem.

Consequences of Premature ejaculation (PE):

What is Premature Ejaculation? 

Premature (early) ejaculation also referred to as rapid ejaculation; also called as early discharge. 
PE is the most common type of sexual dysfunction in men younger than 40 years and seen across all age groups. In elderly men (>60 years) erectile dysfunction is more common. A study conducted by the Journal of Sexual Medicine revealed that this condition is almost equally prevalent in sexually active men of all age groups. 
If a man experiences uncontrollable ejaculation (discharge of semen from your penis) either before or shortly after sexual penetration (usually within 1-2 minutes of penetration), he may be suffering from PE. The Mayo clinic at the United States defines PE as a condition in which  a man  always or nearly always ejaculates within one minute of penetration."

Instances of PE:
An occasional instance of premature ejaculation might not be cause of concern, but, if the problem occurs more than 50% of attempted sexual relations signifies existence of abnormal pattern for which treatment is required.

Premature ejaculation takes place with negligible sexual stimulation and even before the desire 
to ejaculate. Another way of describing PE by most therapist or clinicians is the occurrence of ejaculation sooner during sexual intercourse than he or his partner would like. 

This broad definition thus avoids specifying a precise  normal  duration for sexual activity and reaching a climax. The duration of sexual activity is highly variable and depends on various factors specific to each individual. Take a situation, a male may reach orgasm after 7 to 8 minutes of sexual intercourse, but if his partner is able to achieve climax mostly in 5 minutes and both are satisfied with this timing, then this is not defined as PE. 

Alternatively, a male might delay his ejaculation for up to 15- 20 minutes of sexual intercourse, but if his partner, even with foreplay, requires 25-30 minutes of total sexual activity before reaching climax, he may still consider his ejaculation premature because his partner has not been satisfied (at least, not through intercourse). But in routine practice this kind of situation is NOT called as PE. So every individual and every couple is unique. Same individual may have different sexual behaviour with different partners and different circumstances. Varied situations and varied places of performing the act have a vital role to play in determining the outcomes of individual sexual performance/s.



Knowing Premature Ejaculation - PART 1: Understanding What is PE

Vaginitis also known as vaginal infection or vulvovaginitis, it is a situation in which the vagina gets sore and irritated. It is a common occurrence in females. Though it is not a very serious disorder yet it can be a cause of severe discomfort and irritation. Common side effects that are prevalent in the case of vaginitis include either abnormal discharge or vulvovaginal irritation or both. The discharge may also tend to have an unpleasant odor. In ayurveda, vaginitis is often seen as an amalgamation of yoni dahan and yoni kandu, the two different disorders that affects vagina.

The most common causes of vaginitis include:

Chlamydia is a STD, caused due to intercourse, or even sexual contact. This is often accompanied by vaginal discharge or genital pain. Chlamydia can affect both the sexes to any age group, though young sexually active women are mostly diagnosed with it. If left untreated, this STD can cause further complications, some of them, fatal.

Symptoms of Chlamydia in women
1. Abnormal odorous vaginal discharge
2. Bleeding resulting from intercourse or even between periods
3. Pain during menstruation or sexual intercourse
4. Soreness in the lower abdomen

7. Green or yellow vaginal discharge

Further complications
Infection can extend to the fallopian tubes, leading to a disease called Pelvic Inflammatory Disease (PID). PID can bear symptoms such as nausea, severe pain in the pelvic region, fever and unusual bleeding during one s periods. There are also high chances of contracting the chlamydia infection in the anus, the indications being pain, bleeding and discharge from the anus. Oral sex with a person diagnosed with this condition can also result in you contracting this infection in the throat, resulting in soreness, fever and cough.

Causes of Chlamydia in women
1. Chlamydia, being a sexually transmitted infection, primarily spreads through unprotected sex. However, sexual intercourse need not always be the reason behind this disorder. Sexual contact or even indulging in oral sex without using protection (in the form of dental dams) can result in this disease.
2. Newborn infants are also prone to acquiring this infection that is passed on to them by their mothers during birth. Therefore, pre-natal checkups are a must for detection.
3. Rarely, one can contract the infection in the eyes as well, through any form of oral or genital contact.

This can help in reducing morbidity and improves prognosis. The oral lesions that occur in HIV patients can vary and differ significantly in children and adults. While there are a variety of oral lesions in HIV-infected individuals, listed below are some common infections seen in HIV patients. These are a combination of fungal, viral and bacterial infections.

Candidiasis: Candida is an opportunistic fungus that is normally present in the oral cavity and with reduced immunity of HIV, recurrent bouts of the infection begins to show up. It can be in the form of regular thrush which is whitish and cannot be scraped off (pseudomembranous candidiasis), hyperplastic candidiasis (white patches which can be scraped off) or erythematous (reddish patches). Candida can involve any part of the oral mucosa including the pharynx and the palate.
Herpes Simplex: This is the most common viral infection seen in patients with HIV/AIDS. There could be primary or secondary infection of herpes virus, especially inside the mouth and the vermillion border of the lips.
Herpes zoster: This virus, when already present in the body, can be reactivated with HIV/AIDS and with oral herpes. The distinction with herpes simplex is from their distribution. These are unilateral, along the distribution of the maxillary or mandibular nerve. The lesions appear both on the facial skin and the oral mucosa. While the facial ones break open and form crusts, the mucosal ones coalesce to form larger lesions.
Hairy Leukoplakia: This is present in about 20% of asymptomatic HIV patients. Onset of hairy leukoplakia is an indication of rapid progression of HIV with increased CD4 counts. The typical lesion is a non-movable, hairy lesion along the side of the tongue and can spread to the top and the undersurface of the tongue. There are large amounts of Epstein-Barr virus (EBV) identified from biopsies of hairy leukoplakia.
Cytomegalovirus: If the ulcers have a necrotic base with a halo surrounding it, it is CMV infection, usually seen on any oral mucosal surface.
Periodontal disease: This is one of the bacterial infections that manifests itself in HIV patients. It can take two forms such as Linear Gingival Erythema (LGE) which can subsequently lead to Necrotizing Ulcerative Periodontitis (NUP). The oral hygiene is generally good with minimal plaque and there is rapid bone loss and soft tissue reddening and swelling. The, mouth, therefore is certainly a window to one s health.
