While cold and cough are common, diseases like Addison s disease are rare. It is also known as chronic adrenal insufficiency or hypocortisolism as it is a result of insufficient production of hormones like cortisol and aldosterone by the adrenal glands. This disease can affect men and women of any age.

There are two types of Addison s disease. These are:

    Primary adrenal insufficiency: In this case, the adrenal glands themselves have a problem.
    Secondary adrenal insufficiency: If the hormone production in the adrenal glands is restricted by a problem that has started somewhere else like the pituitary gland, for example, it is known as secondary adrenal insufficiency.

Primary adrenal insufficiency is more common of the two. This could be triggered as a result of autoimmune diseases or a long lasting infection such as HIV, tuberculosis and some fungal infections. Cancer can also cause this type of Addison s disease.

Secondary adrenal insufficiency is usually caused by a problem with the pituitary gland or the hypothalamus as they are responsible for the production of hormones that stimulate the adrenal glands. Damage caused to the pituitary gland by tumors or radiation and surgery can also interfere with hormone production. Prolonged and improper use of steroid hormones can also trigger this condition.

Addison s disease has a very slow progression and its symptoms develop gradually over time. Some of these symptoms are:

    Weight loss and loss of appetite
    Extreme fatigue
    Hyperpigmentation
    Low blood pressure
    Craving for salt
    Nausea and diarrhea
    Pain in the abdomen and joint pains
    Irritability
    Depression
    Loss of body hair and sexual dysfunction in women

Treatment for Addison s disease involves hormone replacement therapy. This aims at correcting the hormone levels in the body. Hormone replacement therapy includes oral corticosteroids and corticosteroid injections. The dosage of these steroids could vary depending on the situation. For example, the doctor may suggest a temporary high dosage when the patient is in a stressful condition. Along with this, sodium is also recommended especially during the summer months and while exercising. This medication must be taken regularly as skipping a single dose can be dangerous.

In some cases this condition can become life threatening. An addisonian crisis causes a drop in blood pressure, sugar and a spike in potassium levels in the body. This requires immediate medical attention and can be treated with intravenous injections of hydrocortisone, saline solution and sugar.

    Exercise: Include 30 minutes of regular exercise into your daily routine if you have a diabetic predisposition.

    Others: Avoid smoking and alcohol, ensure sufficient sleep, avoid sleeping during day time, improved foot and eye care, periodically check sugar levels and manage stress levels.

Medication

Ayurveda has a whole lot of home remedies which have proven to be very effective against diabetes:

    Jambhul: Eugenia Jambolana whether eaten raw or juice extract has been shown to have beneficial effects in managing sugar levels and cholesterol.

    Gymnema sylvestre: Used to manage diabetes for over 2000 years, it reduces sugar cravings and is being touted as the future for diabetes treatment.

    Bitter gourd (Momordica charantia): It has 3 components that give it strong antidiabetic properties. The charantin reduces blood sugar levels; the polypeptide has insulin-like effects; and lectin that again has hypoglycemic effects.

    Bel (Aegle marmelos): Also known as wood apple, leaves of the plant are shown to have antidiabetic properties. 5 to 10 leaves can be chewed on a daily basis to help control blood sugar levels.

    Fenugreek (Trigonella foenum graecum): Consuming 10 g of fenugreek seeds soaked in water increases the amount of insulin produced, thereby increasing sugar breakdown.

    Neem: Chewing about 4 to 5 leaves in the morning on an empty stomach helps to control blood sugar levels. Alternately, neem leaves powder is available which can be dissolved in water and consumed.

Infertility treatment depends on the cause identified by the above tests and hence it is important for both partners to undergo complete testing.

What are the risk factors?
About 2-10% of pregnant women are affected by gestational diabetes every year. The following factors are likely to put you at risk of developing gestational diabetes-

Excess body weight- You may develop gestational diabetes if you were overweight or had gained excess weight (BMI equal to or greater than 30) before getting pregnant
Family history- Women who have a family history of diabetes are likely to have gestational diabetes.
Age- Women aged more than 25 years are at a greater risk of developing this condition.
Diabetic history- A slightly raised blood sugar level may act as the precursor to Type-2 diabetes, which in turn increases your chance of getting affected by gestational diabetes.
Previous pregnancy- Women who ve previously given birth to a child weighing more than 4.1 Kg (9 pounds), or have delivered a stillborn child are likely to have gestational diabetes.
What is the treatment for gestational diabetes?
Urine tests are performed to check for ketones. In a non-diabetic person, glucagon, insulin and other hormones avert ketone levels in your blood from soaring up too high. Once you re diagnosed with gestational diabetes, it s important that you consult your doctor for treatment. Uncontrolled blood sugar level can affect you and your baby s health after birth. This condition can be treated in the following ways-

Keep a check on your blood sugar- During pregnancy, your doctor will have you tested for gestational diabetes 4-5times a day to keep a tab on your blood glucose level. For this, a small drop of blood is first drawn from the finger using a needle. The sample of blood is then placed on the test strip inserted into a device called the blood glucose meter to measure and monitor the level of blood sugar in your body.
Medication- Anti-diabetic medications and insulin are prescribed to control the amount of glucose or sugar in the bloodstream. Your doctor may also suggest oral medications like Diabeta and Glynase to manage your blood glucose level.
Healthy diet- Diabetic diet- one that helps control blood sugar levels- is highly prescribed by doctors during pregnancy. Cut down on carbohydrates and sugar, and switch to food items that provide your body sufficient nutrients and calories.
Post treatment the blood sugar levels usually come down to normal within 4-6 weeks of giving birth.
