Dear doctor I am 32 years old and recently started lower back pain during my regular walk. On consulting orthopaedic doctor dr. Said x-ray shows normal nothing to worry. But I still feel the problem. Any suggestions what could be the problem or something to get relief. Note: I weight 97 kgs and height is 5' 7. Thanks.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young and should not be having regular back pains but it's well known that our spine can have certain issues which leads to these painful attacks. From being just a simple muscular strain to a degenerative disc issue, it can be anything. My advise to you seeing your age and symptoms only localized to your lower back region with no radiculopathy or any other neurological symptoms, would be: - catch hold of a good physical therapist, start your back muscle training & strengthening, abdominal strengthening and general conditioning ex - take adequate care of your posture especially while at work, home etc - continue with your brisk walks initially for a less distance and increase it gradually. - you can take anti-inflammatory medication which suits you for 5-7 days and it should give you adequate pain relief. - x rays normally do not show anything unless there is a bony pathology in spine, hence to actually see whats wrong, it is better to get mri done, but maybe do your physical therapy well in pain free periods, and you may become better gradually. For leg pain, please follow the below advise: keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.

Since your blood sugars are high. You got balanitis (fore skin infection. You have to bring your sugars under control and has to take antibiotics and local applications. Then it will settle.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. There are many causes of pain in neck region. It can be due to faulty posture, related to weak muscles, issues with intervertebral disc, narrowing of spinal canal, any bony instability besides many other, so accurate diagnosis is the key to proper management of the condition. You will have to elaborate your problem as to a detailed discussion of your neck pain which means duration of pain and mode of onset. I would also like to ask you if it is associated with any radiating upper limb pain or any other neurological symptoms like numbness, paraesthesias (electrical shock like abnormal sensations) or any weakness in limbs. Please also tell me about any associated symptoms like fever, weight loss etc. What medications you generally take and what has been your treatment history till now for existing problem. I will also like to see your investigations like x rays and mri scan of the cervical spine so that I can identify the pathology and advise you accordingly. Leg exercise kijiye daily I hope you will fit and fine………………………………………………. Basic Leg Exercises The following Basic Leg Exercises should generally be performed 1 - 3 times per week provided they do not cause or increase pain. Ideally they should not be performed on consecutive days, to allow muscle recovery. As your strength improves, the exercises can be progressed by gradually increasing the repetitions, number of sets or resistance of the leg exercises provided they do not cause or increase pain. Swiss Ball Squats Begin this leg exercise in standing with your feet shoulder width apart, your feet facing forwards and a Swiss Ball placed between a wall and your back (figure 1). Slowly perform a squat, keeping your back straight. Your knees should be in line with your middle toes and should not move forward past your toes. Then slowly return to the starting position maintaining good posture throughout the exercise. Perform 1 - 3 sets of 10 repetitions provided the exercise is pain free Lunges Begin this leg exercise standing with your back straight in the position demonstrated (figure 2). Slowly lower your body until your front knee is at a right angle. Keep your front knee in line with your middle toe and your feet facing forward. Then slowly return to the starting position maintaining good posture throughout the exercise. Perform 1 - 3 sets of 10 repetitions provided the exercise is pain free Hip Abduction Sidelying Begin this leg exercise lying on your side in the position demonstrated (figure 3). Keeping your back and knee straight and foot facing forwards, slowly take your leg upwards tightening the muscles at the side of your thigh / hip (abductors). Hold for 2 seconds and then return to the starting position. Perform 1 – 3 sets of 10 repetitions on each leg provided the exercise is pain free. This exercise can be progressed by adding an ankle weight to your ankle for increased resistance.

Good exercises, vitamin d rich food and exposure to sun for atleast 15 minutes a day will improve the vitamin d level will help you in recovering from your present complains.

Apply hot fomentation twice daily. Avoid bending in front. Postural correction- sit tall, walk tall. Extension exercises x 15 times x twice daily. Bhujang asana. Core strengthening exercises. Back stretching. Do the cat/cow stretch.

Back pain could be mechanical basically due to long sitting hours. So you should get up every 2-3 hrs from the chair and do back stretching or extension exercises, also neck stretching. Besides that ergonomic chairs should be used and proper posture should be maintained while working, avoid slouching. You can learn core strengthening and back extension exercises from a physiotherapist and do it every day twice, this will help in strengthening your back muscles.

I am 36 years old male suffering with hypermobility of joints. It was diagnosed 2 years ago with stiffness and pain in lower back and sacroiliac joints. My joints are very weak and get hurt easily since my childhood. Its been more than a month that I took stairs to office for a week instead of lift. I used to climb 15 floors without lift. My knees started paining and then I stopped doing it. Now my knees are swollen and they are not getting better even after icing or using hot water bottle. All the tests in the past were negative. The tests done were RA Factor, HLAB27, BL17, CRP, cbc, MRI and complete spine screening and Bone scan of lower back. My lower back is fine now but I have swelling in my knees and pain in left shoulder and left wrist.

Identify the mid point between the ankle bone and tendons and give pressure with a blunt edge on either side of your foot for 2 minutes. Also give pressure in the web between 2nd and 3rd finger from your toe for 2 minutes. Finally give a good massage for both feet in hot water. Identify the dipping portion on the back of your knee folding. Press this dip with both middle fingers and hold your knee with your thumbs for support. Please swing 20 times morning and evenings. Inform after 10 days for further guidance.

1. Do warm saline gurgling 3-4 times daily 2. Take viscodyne d syrup 2tsf thrice daily if it is a dry cough or viscodyne syrup if cough is associated with sputum 3. You may have to take antibiotic along with it after consulting doctor, if no relief4. Avoid smoking if you smoke.

Tk calcium and do hot fermentation and avoid strain for 2 days. Apply pain relief ointment. Put a towel roll under knee and press for 10 sec then release repeat 10 time. And quadriceps exercise in gradually process for strength.

Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day. Quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain.
